摘要
目的通过对脑卒中患者家庭护理质量的评价,了解患者各方面需求的满足程度,并对影响因素进行分析,为进一步的护理干预提供依据。方法采用问卷对268例脑卒中患者进行调查,内容包括患者一般情况、照顾者一般情况、脑卒中患者家庭护理质量状况、照顾者负担状况。结果 66.0%的照顾者负担分值为大于40~60分,处在中到重度水平。脑卒中患者家庭护理质量5个维度中,生理需求的满足得分最高,自我实现需求的满足得分最低。采用多元逐步回归分析,患者年龄、患者文化程度、积蓄或离退休金能否满足治疗和生活支出、照顾者是否接受过相关的照顾指导和培训、协助照顾人数、照顾者的负担状况7个因素对家庭护理质量得分有显著性影响。7个影响因素中,患者年龄和照顾者负担对家庭护理质量得分的影响是负向的,其余5个的影响是正向的。结论脑卒中患者家庭护理质量的影响因素较多,可以从患者的年龄、患者文化程度、经济状况、照顾者健康状况、照顾者是否接受过相关的照顾指导和培训、协助照顾人数、照顾者的负担状况入手,制定脑卒中患者家庭护理干预措施,以提高家庭护理质量,改善患者的生活质量。
Objective To study the patients′ satisfaction with the various aspects demand through the evaluation of the quality of home care patients with stroke. Through the study of factors affecting to provide the basis for further nursing intervention.Methods Among 268 stroke patients that patient general condition,caregivers general situation,the condition of stroke patients quality of home care,caregiving burden were investigated by a questionnaire survey. Results 66.0% of caregivers′ caregiving burden score was more than 40-60 scores,it was in the medium to heavy level.Among the quality of home care with stroke pa-tients′ 5 dimensions,the satisfaction of physiological needs had the highest score,self-actualization needs had the lowest score.It showed by multiple stepwise regression analysis that the quality of home care were related to the age,cultural degree of patients,savings or retired gold meeting the treatment and living expenses,the caregiver whether to accept guidance and training of health care,co-caregiver quantity,caregiving burden condition. Among 7 factors,patients′ age and caregiving burden condition of quality of home care score were negative correlation,the other 5 influencing factors were positive correlation. Conclusions The influencing factors of the quality of home care with stroke patients are more,in order to improve the quality of family care and patient＇s quality of life,we can formulate measures of home nursing intervention in stroke patients though those aspects,which the age,cultural degree,economic condition of patients,health condition of the caregiver,the caregiver whether to accept guidance and training of health care,co-caregiver quantity,caregiving burden condition.
