探讨X线平片、CT及磁共振成像对脊椎骨折的不同诊断价值 被引量:12

摘要 目的探讨并比较脊椎骨折临床检查中X线平片、CT、磁共振成像(MRI)3种影像学工具各自的优缺点及其诊断价值。方法收集78例病历资料,伤患病情程度不同,但均进行了以上3项检查。对骨折数、脊椎曲型异常数、骨碎片数、椎管容积异常数、脊髓损伤数、骨挫伤数、韧带损伤数、神经压迫或受损数及周围组织损伤数等情况进行考察和比对,考察3种影像学工具的诊断效果。结果 X线平片、CT、MRI分别检出椎骨受损70、72、78例,检出脊柱变形均为68例,检出骨碎片分别为38、208、174块,在脊髓、神经根及韧带等部位检测中,三者结果差异均有统计学意义(P<0.05)。结论 X线平片用于大范围初步检查,CT检查骨碎片最细致,MRI用于骨周围软组织、神经和局部水肿等检查。 【Objective】To investigate and compare the advantages, disadvantages and diagnostic value of three kinds of imaging tools such as X-ray plain film, computed tomography(CT) and magnetic resonance imaging(MRI) in the clinical examination of spinal fractures.【Methods】Medical records of 78 cases were collected. The degree of injury was different, but all of the above three tests were carried out on them. The fracture number, abnormal number of vertebral curvature, the number of abnormal bone fragments, abnormal volume of vertebral canal, the number of spinal cord injury and bone contusion and ligament injury, nerve compression or impairment and surrounding tissue damage number etc. were studied and compared. The diagnostic effect of three imaging tools was investigated.【Results】Vertebral injury was detected in 70 cases, 72 cases and 78 cases respectively by X-ray, CT and MRI, all 68 cases of spinal deformity were detected, and the numbers of bone fragments detected were respectively 38, 208 and 174. In the detection of spinal cord, nerve root and ligament, the results of the three groups were significantly different(P〈0.05).【Conclusion】X-ray plain film is used for the preliminary examination of a large scale. CT examination shows the most detailed bone fragments. MRI is used for the examination of soft tissue around the bone, nerve and local edema.
