摘要
目的分析比较X线平片、螺旋CT、MRI检查技术的在脊椎骨折诊断中的作用。方法收集89例脊椎骨折的X线平片、螺旋CT、MRI影像学资料,对其影像学表现进行回顾性分析。结果所有89例椎体骨折病例的均有脊柱曲度改变,X线平片、螺旋CT及MRI均能全部显示;对椎体骨折线、椎弓骨折的显示,CT轴位图像结合MPR及3D重建、X线平片和MRI的显示率分别为71%、32%、63%,CT优于X线平片和MRI;MRI显示脊髓损伤、韧带损伤、椎体骨髓挫伤(83%)比螺旋CT(68%)具有优势。结论 X线平片是急性脊柱骨折的基础检查方法;螺旋CT的横轴位、多平面重建及3D联合运用能明确损伤部位、程度,立体直观,有利于临床手术方式的选择;MRI对于脊髓损伤的改变和骨挫伤的显示良好。
Objective To study the X-ray plain radiography,spiral CT and MRI manifestations and their diagnostic value of vertebrae fracture.Methods The X-ray plain radiography,spiral CT and MR imaging data of 89 cases of vertebrae fracture were analyzed retrospectively.Results The change of spine curvature of 89 cases of vertebral fracture were shown in all X-ray plain radiography,spiral CT and MR imaging.The axial CT scan image combine with MRP and 3D reconstruction was better than X-ray and MRI at showing vertebral fracture line and vertebral arch fracture,their rate of showing were 71%,32%,63%,respectively.But MRI was better than spiral CT at showing spinal cord ligament injury and vertebral bone contusion,their rate of showing were 83%,68%,respectively.Conclusion X-ray plain radiography is the fundamental method to find acute veterbral fracture.The axial spiral CT scan and multiplan reconstruction combine with 3D reconstruction can definitely,stereoscopically and directly demonstrate the degree and severity of the fracture and can help to select the surgical way.MRI is the best method to demonstrate the spinal cord injury and bone contusion.
