尿液巨细胞病毒核酸检测结合全血细胞计数在小儿巨细胞病毒感染诊断中的应用 被引量:2

摘要 目的调查儿童人巨细胞病毒(HCMV)感染状况,探讨HCMVDNA、白细胞计数与分类的联合检测在小儿HCMV感染诊断与治疗中的应用价值。方法采用FQ—PCR方法检测518例不明原因支气管炎、上呼吸道疾病、发热等患儿尿液中的HCMVDNA含量,同时采集EDTA抗凝全血,进行全血细胞计数,统计分析患儿具体信息、诊断和检测结果。结果HCMVDNA阳性率49.2%,男患儿在1-≤3岁年龄段的检出率最高,女患儿阳性检出率则随年龄增长逐年下降;淋巴细胞计数与比例、单核细胞计数在治疗前后与正常对照比较,差异有统计学意义(P〈0.01),而中性粒细胞比例则较正常对照显著下降(P〈0.01);儿童HCMV感染主要累及肺脏,引起疾病复杂多样并以呼吸道疾病为主;经1周治疗后,HCMVDNA病毒载量与发病初期比较,差异有统计学意义(P〈0.01)。结论不同年龄纽儿童间HCMV感染率不同,HCMV感染与性别有关,HCMV感染可导致WBC数量异常增高,尿HCMVDNA定量测定与WBC计数和分类的联合检测在不明原因呼吸道感染患儿的早期病因诊断与抗病毒治疗动态监测及疗效判断中有参考价值。 Objective To investigate the status of HCMV infection in children, combined detection of DNA and white blood cell count and classification in the application value in the diagnosis and treatment of HCMV infection in children. Methods HCMV DNA content in urine of 518 patients with unexplained bronchitis, upper respiratory tract disease and fever were detected by FQ-PCR. EDTA anticoagulant blood was collected for CBC, analysis of children with specific information, diagnostic and statistical results. Results The positive rate of HCMV DNA was 49.2%, the male patients in the highest detection rate of 1 ~ is less than or equal to 3 years of age, the positive rate of female children decreased year by year; compared with the control group, there was significant difference in the percentage of lymphocyte count and the proportion of mononuclear cells ( P〈0.01 ) , while the proportion of neutrophils decreased significantly (P〈0.01) . HCMV infection in children was mainly involved in the lung, resulted in a variety of diseases, including respiratory diseases; after a week of treatment, HCMV DNA viral load compared with the initial stage, the difference was statistically significant ( P〈0.01 ) . Conclusion HCMV infection rates are different among children of different age groups, HCMV infection is related to sex. HCMV infection can lead to abnormal increase in the number of WBC, the quantitative detection of urinary HCMV DNA combine the detection of WBC count and classification in the early diagnosis of children with respiratory infection of unknown origin have reference value, also take effect for the dynamic monitoring and evaluation of therapeutic efficacy of antiviral therapy.
