摘要
目的研究脑白质疏松对遗忘型轻度认知障碍患者(a MCI)认知功能和日常生活能力的影响。方法对67例a MCI患者(a MCI组)和71名正常志愿者(正常对照组)进行临床资料采集。采用MMSE、Mattis痴呆评定量表(MDRS)、听觉词语学习记忆量表(AVLT)和日常生活能力评估量表(ADL)分别评估受试者认知功能和日常生活能力。采用Fazekas评分方法对脑室旁白质高信号(PVH)和深部白质高信号(DWMH)独立评分。结果 a MCI组ADL评分、PVH评分和DWMH评分均显著高于正常对照组(均P<0.05)。a MCI组MMSE评分、MDRS-总分、MDRS-起始/保持评分、MDRS-概念形成评分、MDRS-记忆评分、AVLT-短延迟记忆评分、AVLT-长延迟记忆评分均明显低于正常对照组(均P<0.05)。多元逐步回归分析结果显示,MDRS-起始/保持评分为a MCI组患者ADL评分的影响因素(β=-0.743,P<0.05)。年龄和DWMH评分分别为a MCI组患者MDRS-起始/保持评分的影响因素(β=-0.662,P<0.05;β=-0.228,P<0.05)。结论 a MCI患者深部脑白质疏松越严重、年龄越大,其执行功能损害越严重,可导致日常生活能力下降。
Objective To investigate the influence of leukoaraiosis on cognitive function and activities of daily life in patients with amnestic mild cognitive impairment（ a MCI）. Methods The clinical data of 67 a MCI patients（ a MCI group） and 71 normal control subjects（ normal control group） were collected. Their cognitive function and ADL were examined by MMSE,mattis dementia rating scale（ MDRS）,auditory verbal learning test（ AVLT） and activity of daily living scale（ ADL）. Periventricular hyperintensities（ PVH） and deep white matter hyperintense signals（ DWMH） were scored by Fazekas method. Results The ADL score,PVH score and DWMH score of a MCI group were significantly higher than those in normal control group（ all P〈0. 05）. The MMSE score,MDRS-total score,MDRS-start/maintain score, MDRS-concept formation score, MDRS-memory score, AVLT-short latency memory score and AVLT-long latency memory score of a MCI group were significantly lower than those in normal control group（ all P〈0. 05）. Multiple stepwise regression analysis showed that MDRS-start/maintain score was the influence factor of the ADL score in a MCI patients（ β =-0. 743,P〈0. 05）; Age and DWMH score were the influence factors of the MDRS-start/maintain score in a MCI patients（ β =-0. 662,P〈0. 05; β =-0. 228,P〈0. 05）. Conclusion The worse deep leukoaraiosis and the older age of a MCI patients,the more serious the executive function is. And it can result in the decrease of daily life abilities in a MCI patients.
