彩色多普勒超声血流阻力指数联合CA125检测对卵巢肿瘤的诊断价值 被引量:8

摘要 目的:探讨彩色多普勒超声血流阻力指数(resistance index,RI)联合血清肿瘤标记物糖链抗原125(CA125)对卵巢肿瘤的诊断价值。方法:选取经病理证实的良性卵巢肿瘤50例,恶性卵巢肿瘤42例,比较良、恶性卵巢肿瘤超声声像图表现、血流信号、RI及CA125水平,分析RI、CA125单独及联合检测卵巢肿瘤的诊断价值。结果:良性卵巢肿瘤超声多表现为囊性回声,恶性则多表现为囊实性混合回声。良、恶性卵巢肿瘤血流率分别为28.0%和85.7%,两者比较差异有统计学意义(P<0.01)。恶性卵巢肿瘤RI值显著低于良性卵巢肿瘤(P<0.01),但CA125水平明显高于良性卵巢肿瘤(P<0.01)。RI联合CA125检测卵巢肿瘤的敏感度和特异度均高于RI、CA125单独检测。结论:彩色多普勒超声血流RI联合血清CA125检测在鉴别诊断良、恶性卵巢肿瘤方面优于RI、CA125单独检测。 Objective:To explore the value of color Doppler ultrasound resistance index (RI) combined with serum tumor marker carbohydrate antigen 125 (CA125) in the diagnosis of ovarian tumor.Methods:Totally 50 cases of benign ovarian tumor and 42 cases of malignant ovarian tumor confirmed by pathology were selected.The ultrasonogram manifestations,blood flow signals,RI and CA125 levels of benign and malignant ovarian tumors were all compared,and the diagnostic values of RI alone,CA125 alone as well as RI combined with CA125 were analyzed.Results:The ultrasonogram of benign ovarian tumor manifested cystic echo,while that of malignant ovarian tumor manifested solid-cystic echo.The blood flow rates of benign and malignant ovarian tumors were 28.0% and 85.7%,respectively,and the difference was statistically significant (P〈0.01).The RI of malignant ovarian tumor was markedly lower than that of benign ovarian tumor (P〈0.01),whereas CA125 level was significantly higher than that of benign ovarian tumor (P〈0.01).In the detection of ovarian tumor,RI combined with CA125 had higher sensitivity and specificity when compared with RI alone or CA125 alone.Conclusion:Color Doppler ultrasound RI combined with serum CA125 is superior to RI alone or CA125 alone in the differentiation and diagnosis of benign and malignant ovarian tumors.
