摘要
目的:比较分析甩肩疗法与小夹板固定疗法对女性肱骨近端骨折NeerⅢ型的临床疗效。方法:选取辽宁中医药大学附属医院2016年1月至2016年12月收治的肱骨近端骨折女性患者20例,随机分为2组,各10例,对照组行小夹板固定治疗,观察组行甩肩疗法治疗。治疗后行X线检查肱骨复位情况,考察治疗效果和肩关节功能评分(Constant评分)。结果:2组术后骨折线对位情况等临床疗效比较差异无统计学意义(P>0.05);2组患者Constant评分:关节活动度评分(ROM评分)比较差异无统计学意义(P>0.05);疼痛评分、日常生活活动能力评分(ADL评分)、肌力评分(MMT评分)、Constant总评分比较,观察组均高于对照组,差异均有统计学意义(P<0.05)。结论:2种疗法对于肱骨近端骨折NeerⅢ型的治疗疗效无明显差异,但甩肩疗法在术后3个月内肩关节功能方面优于小夹板固定疗法。
Objective：To compare the clinical effect between shoulder swinging and small splint fixation in the treatment of females with proximal humeral fractures type NeerIII. Methods： Twenty female patients with proximal humeral fracture treated in our hospital from Jan 2016 to Dec 2016 were enrolled in this study. These patients were randomly divided into the observation group underwent shoulder swinging and the control group underwent small splint fixation, 10 cases in each group. After treatment, the humeral fracture reduction was examined by X-ray, and the clinical effects and Constant shoulder scores between the two groups were compared. Results： There was no significant difference between the two groups in the curative effect of humeral fracture reduction （p〉0.05）. The range of motion （ROM） score of the control group was slightly lower than that of the observation group, but the difference was not statistically significant （p〉0.05）. However, the pain score, ADL score, MMT score and Constant total score of the observation group were higher than those of the control group, the difference was statistically significant （p〈0.05）. Conclusions： There is no significant difference between the two therapies in the treatment of proximal humerus fractures type NeerIII. However, shoulder swinging is superior to small splint fixation in shoulder function within 3 months after surgery.
