摘要
目的探讨经皮膀胱微穿刺引流辅助经尿道输尿管镜2μm激光碎石术治疗小儿膀胱结石的可行性。方法对10例接受经尿道输尿管镜2μm激光碎石术(对照组)和16例同时接受经皮膀胱微穿刺引流(观察组)的小儿膀胱结石患者进行回顾分析,比较两组手术时间、留置导尿时间、停导尿后排石时间、术后住院日和尿道狭窄发生率。结果观察组手术时间、留置导尿时间和术后住院日明显短于对照组(P<0.05),两组停导尿后排石时间和尿道狭窄发生率比较,差异无统计学意义(P>0.05)。结论经尿道输尿管镜2μm激光碎石术是治疗小儿膀胱结石的一种安全可靠的微创手术方法 ,辅以经皮膀胱微穿刺引流可在不增加手术创伤的情况下缩短手术时间,加快康复。
Objective To explore the feasibility and effect of percutaneous bladder micro-puncture drainage combined with transurethral 2-μm thulium laser lithotripsy on treatment of pediatric bladder calculus. Methods A retrospective analysis was made between two groups： the control group and the observation group. In control group, 10 patients received transurethral 2 μm thulium laser lithotripsy. And 16 patients received percutaneous bladder micro-puncture drainage combined with transurethral 2-μm thulium laser lithotripsy. Compare the operation time,indwelling catheter time,stone clearance after removal catheter time,hospitalized time after operation,and the incidence of postoperative urethral stricture between the two groups. Results The operation time,indwelling catheter time and hospitalized time after operation were significantly shorter in observation group than control group（P〈0.05）. And there was no obvious difference of stone clearance after removal catheter time,and the incidence of postoperative urethral stricture between these 2 groups（P〉0.05）. Conclusion Transurethral 2μm thulium laser lithotripsy by ureteroscopy is a superior microinvasive surgery method for pediatrics with bladder calculi,which combined with little blood loss,high safety,convenient operation and infrequent complications. And percutaneous puncture drainage may optimize this technique formerly by shorting operation time and accelerating the course of convalescence.
