摘要
目的探讨超声弹性成像、常规超声及两者联合在乳腺肿块定性诊断中的作用。方法选择因乳腺肿块在我院治疗的300例女性患者为研究对象(共计327个肿块),所有患者术前均采用超声弹性成像、常规超声检查,手术后肿块经病理证实。结果常规超声检测乳腺肿块性质的敏感度为81.08%,特异度为80.99%,符合率为81.04%;超声弹性成像技术检测乳腺肿块性质的敏感度为87.03%,特异度为88.73%,符合率为87.76%;联合检测乳腺肿块性质的敏感度为91.35%,特异度为92.25%,符合率为91.74%;三种诊断方法敏感性、特异性与符合率比较差异有统计学意义(P<0.05),其中联合诊断敏感性、特异性与符合率均比常规超声以及超声弹性成像高。结论超声弹性成像、常规超声联合可达到互补,提高诊断敏感性、特异性及准确度,对乳腺肿块的早期诊断、治疗具有重要意义。
Objective To investigate the role of ultrasound elastography,conventional ultrasound and combination of them in the qualitative diagnosis of breast masses. Methods 300 cases of female patients treated with breast lumps in our hospital were selected as the subjects （total of 327 lumps）. All patients underwent ultrasonic elastographyand routine ultrasound examination before operation. The postoperative mass was confirmed by pathology. Results Ultrasound detection of breast masses sensitivity was 81.08%,specificity was 80.99%,the coincidence rate was 81.04%. Ultrasonic elastography of breast masses detection sensitivity was 87.03%,specificity was 88.73%,the coincidence rate was 87.76%. The combined detection of breast masses sensitivity was 91.35%,specificity was 92.25%,the coincidence rate was 91.74%. The sensitivity,specificity and specificity of the three diagnostic methods were significantly different （P 〈 0.05）, the diagnostic sensitivity,specificity and coincidence rate in combined detection were higher than those of ultrasound and ultrasound elasticity imaging. Conclusion Ultrasound elastography combined with conventional ultrasound can achieve complementarity,and improve diagnostic sensitivity,specificity and accuracy. It is important for early detection and treatment of breast masses.
