房性心律失常消融与左心耳电隔离

摘要 房性心律失常消融策略根据其机制常涉及线性、局灶及基质改良消融[1-4],消融径路及范围因其不同机制存在较大差异性.左心耳(LAA)因其特殊形态及结构不仅为心房颤动(房颤)血栓形成最主要的部位,也是其发生和维持的关键区域之一,部分房颤患者可经主动左心耳电隔离(left atrial appendage isolation,LAAI)获益[5].另外,复杂性房性心律失常常需于左心房内行激进消融以达到消融终点,使损伤与LAA相关传导束风险增加而易致被动LAAI[3].消融相关的医源性LAAI理论上可使LAA血栓及血栓栓塞事件风险显著增加[6-7],需引起电生理术者高度警惕.
