摘要
目的观察华蟾素注射液联合顺铂胸腔内灌注治疗恶性胸腔积液的临床疗效。方法选取该院收治的肺癌恶性胸腔积液患者64例,采用随机数字表法分为观察组(32例)和对照组(32例),二组患者均接受引流治疗,并在此基础上,观察组患者接受腔内联合灌注华蟾素和顺铂治疗,对照组患者接受腔内单独顺铂灌注治疗。对二组患者的疗效、不良反应以及肿瘤标志物在治疗前后的变化进行评估。结果治疗后二组患者疗效差异明显,观察组患者的有效率为81.3%(26/32),明显高于对照组46.9%(15/32),差异有统计学意义(P<0.05)。治疗后,观察组患者CEA、CA19-9和CA125水平显著低于对照组(P<0.05)。观察组胸痛和骨髓抑制发生率明显低于对照组(P<0.05)。结论华蟾素联合顺铂能有效提高恶性胸腔积液患者的疗效,不良反应轻,同时能降低肿瘤标志物的水平。
Objective To observe the clinical efficacy of using Cinobufacini injections combined with intracavitary perfusion of Cisplatin in the treatment of malignant pleural effusion.Methods Out of the admitted patients diagnosed with malignant pleural effusion,64 cases were randomly selected and divided into an experimental group（32 cases）and a control group（32 cases）.On top of the drainage treatment applied on both groups,patients in the experimental group were treated with intracavitary perfusion of Cisplatin and Cinobufacini injections,while those in the control group were treated with intracavitary perfusion of Cisplatin only.The comparison of efficacy,adverse reactions and variations of tumor-related factors before and after the treatments in two groups were evaluated.Results After treatment,differences were observed in efficacy rates in both groups.The total effective rate for the experimental group was 81.3%,much higher than46.9%of the control group,in which the difference exhibits statistical significance（P〈0.05）.The rate of CEA,CA19-9 and CA125 present,after treatment,was significantly lower than that of the control group（P〈0.05）.In the experimental group,the incidences of stethalgia and myelosuppression were much lower than in the control group（P 〈0.05）.Conclusion Combining Cinobufacini with Cisplatin can effectively improve the clinical efficacy,yield less severe adverse reactions and reduce the presence of tumor-related factors when treating malignant pleural effusion.
