摘要
目的综合评价2013—2015年甘肃省结核病防治工作,为改善今后工作提供依据。方法利用TOPSIS和SWOT法,分析2013—2015中国结核病信息管理系统和甘肃省结核病防治年报、汇编和疫情通报等资料。结果 3年间防治工作质量排名前三位的市(州)分别为陇南市(C_i=0.93)、甘南州(C_i=0.85)和张掖市(C_i=0.59),排名后三位的为兰州市(C_i=0.19)、天水市(C_i=0.25)和定西市(C_i=0.28);甘肃省结核病防治工作的优势为新型结核病防治服务体系的有序推进,不足之处为防治经费投入和人员配置不足、公众肺结核病防治知识知晓率低,机遇为医疗保险保障制度的不断完善,主要威胁为流动人口和耐药肺结核病防治;2015年甘肃全省登记的外地户籍肺结核病病人799例,按照2010年甘肃省结核病流行病学调查显示数据推算,甘肃省耐药肺结核病病人约为0.175万。结论 TOPSIS法结合SWOT法在综合评价地区结核病防治质量方面适用性较好,甘肃省各市(州)结核病防治质量不平衡,优势与不足、机遇与挑战并存。
Objective To comprehensively evaluate tuberculosis control in Gansu province, to provide an evidence for improving future control work. Methods The data from national tuberculosis information management system and annual tuberculosis report, compilation and bulletin of epidemic situation 2013—2015 were analyzed by TOPSIS and SWOT. Results The work qualities of tuberculosis control in Longnan city（C_i=0.93）, Gannan prefecture（C_i=0.85） and Zhangye city（C_i=0.59） were the top 3, while ranked the last three in Lanzhou city（C_i=0.19）, Tianshui city（C_i=0.25） and Dingxi city（C_i=0.28）. The strengths of tuberculosis control of new service system developed in the orderly advancement in Gansu province,the weaknesses were insufficient fund support and staff, the opportunities were the gradual perfection of medical insurance system, and the threats were from the difficulties of multi-drug resistance tuberculosis and migrant tuberculosis control. There were 799 migrant cases with tuberculosis registered in the province in 2015, and there were about 1 750 drug resistance cases according to the epidemic survey of tuberculosis conducted in Gansu province in 2010. Conclusions To generally evaluate the regional tuberculosis control by combining TOPSIS and SWOT is of good applicability. The quality of tuberculosis control presents a situation of unbalanced development, with the co-existence of strengths and weaknesses, opportunities and challenge.
