摘要
2型糖尿病合并冠状动脉粥样硬化性心脏病多为久病痼疾,应属本虚标实。根据临证经验将2型糖尿病分为阴虚火旺兼血瘀证、气阴两虚兼血瘀证和阴阳两虚兼血瘀证。临证时主要从"培肾固本""调和脾胃""交通阴阳"3个方面着手,并辅以通阳散结、活血祛瘀、行气化痰。根据"方证对应"的原则,确定治则为"扶正祛邪,攻补兼施",治法以"养心阴,益心气,培补肾元,宽胸调胃,活血宣痹"为主。针对阴虚火旺兼血瘀证,以六味地黄丸合桃红四物汤加减;气阴两虚兼血瘀证,以生脉汤合补阳还五汤加减;阴阳两虚兼血瘀证,以炙甘草汤合血府逐瘀汤加减。
Type 2 diabetes with coronary atherosclerotic heart disease is mostly a chronic illness,with deficiency syndrome and excess in symptoms. According to the clinical experience,type 2 diabetes can be divided into three categories： Yin Deficiency with Innate Fire with Blood Stasis,Qi and Yin Deficiency with Blood Stasis and Yin and Yang deficiency with Blood Stasis. Treatments of the disease are mainly tonifying Kidney,regulating Spleen and Stomach as well as regulating Yin and Yang,with aspiring Yang and dispelling Stasis,activating Blood circulation,regulating Qi and dispeling phlegm as supplementary treating strategies. According to the principle of prescription and syndrome being corresponding,the treatment policy should be helping the Righteousness and dispelling the Evil and the treatment method are tonifying Heart Yin and Qi,strengthening Kidney and Stomache as well as activating Blood and dispersing rheumatism. As for the Yin Deficiency with Innate Fire with Blood Stasis,modefied Liuwei Dihuang Pill an Taohong Siwu Decoction can be used. As for Qi and Yin Deficiency with Blood Stasis,modified Shengmai Decoction and Buyang Huanwu Decoction can be used. As for Yin and Yang Deficiency with Blood Stasis,modified Zhigancao Decoction and Xuefu Zhuyu Decoction can be used.
