青少年近视眼黄斑参数与等效球镜、眼轴、性别及年龄的相关性分析 被引量:13

摘要 目的探讨青少年近视眼黄斑参数与等效球镜、眼轴、性别、年龄的相关性。方法从陕西省铜川市妇幼保健院眼科2014年3月至2015年3月就诊的青少年中选取屈光度在-0.75^-12.00 D,眼轴长度为24~27 mm的近视患者220例,分为轻度近视组、中度近视组和高度近视组。使用光学相干断层扫描仪(OCT)对其黄斑区视网膜平均厚度,黄斑中心凹厚度,黄斑中心区厚度,黄斑内环、外环的颞侧、上方、鼻侧及下方各个区域的厚度,视网膜体积进行测量,分析黄斑参数与等效球镜、眼轴、性别、年龄的相关性。结果三组青少年近视患者眼底黄斑内环上方与内环鼻侧厚度最厚,在黄斑中心区,黄斑内环鼻侧、内环上方、内环颞侧、内环下方及外环鼻侧的厚度差异均有统计学意义(P<0.05)。相关性分析结果显示,黄斑外环颞侧、下方,平均黄斑厚度及黄斑体积与等效球镜呈正相关关系(r=0.154、0.207、0.213、0.170,P<0.05),与眼轴长度呈负相关关系(r=-0.211、-0.213、-0.255、-0.194,P<0.05)。中心凹黄斑厚度与等效球镜之间呈负相关关系(r=-0.201,P<0.05),与眼轴长度呈正相关关系(r=0.238,P<0.05)。结论青少年近视患者,其黄斑参数与等效球镜、眼轴长度密切相关。 OBJECTIVE To study the correlation between the macular parameters and the spherical equivalent,axis, sex and age of juvenile myopia. METHODS From March of 2014 to March of 2015, 220 myopia adolescents visited Shaanxi Tongchuan maternal and child healthcare center whose diopter between-0.75 to 12.00 D and ocular axial length ranged 24 to 27 mm were selected and divided into mild myopia, moderate myopia and severe myopia group. The macular thickness, foveal thickness, the thickness of inferior, superior, temporal and nasal of inner ring and outer ring as well as other regions, together with macula volume were measured with optical coherence tomogra-phy(OCT). And then correlation between macular parameters and spherical equivalent, axis, gender and age were analyzed. RESULTS The thickness of superior of inner ring and nasal of inner ring was the thickest among all parts of macular. In foveal part, there were significant differences in macular thickness between nasal of inner ring, superi-or of inner ring,temporal of inner ring, inferior of inner ring, nasal of outer ring(P〈 0.05). Correlation analysis showed that in temporal of outer ring and inferior of outer ring, there was a positive correlation existed between the average macular thickness and volume and spherical equivalent(r =0.154, 0.207, 0.213, 0.170, P〈 0.05), and a negative correlation with axial length(r=-0.211,-0.213,-0.255,-0.194, P0.05).The foveal thickness showed a negative correlation with spherical equivalent(r =-0.201, P〈 0.05), and a positive correlation with the axial length of the eye(r =0.238, P0.05). CONCLUSIONS The macular parameters of juvenile myopia were closely linked to the spherical equivalent and axial length.
