摘要
目的探讨EB病毒DNA拷贝数与鼻咽癌患者预后的关系。方法应用实时定量PCR的方法检测EB病毒编码BALF5基因在382例鼻咽癌患者组织中的载量,并以EB病毒阳性的鼻咽癌细胞系C666-1作为对照,根据Livak(2-ΔΔCt)计算方法评估鼻咽癌组织中EB病毒DNA的拷贝数;其中拷贝数倍数值≥1.64为高拷贝数组,反之为低拷贝数组。结果 EB病毒DNA拷贝数范围为0~34.78,平均值为1.64。单因素分析结果显示,高拷贝数的患者发生疾病进展的风险(HR=2.06,95%CI:1.01~4.24)和远处转移的风险(HR=3.06,95%CI:1.11~8.46)高于低拷贝数组,差异有统计学意义(P<0.5)。而多因素结果表明EB病毒高拷贝数是鼻咽癌患者疾病进展和远处转移独立的预后因素。结论 EB病毒拷贝数在鼻咽癌患者的肿瘤进展与转移过程中明显增加,因而,通过检测鼻咽癌组织中的EB病毒DNA拷贝数以监测鼻咽癌患者的疾病进展以及肿瘤转移具有重要意义。
Objective To explore the correlation between epstein-bar virus(EBV)copy numbers and prognosis of nasopharyngeal carcinoma. Methods The DNA level of EBV encoded BALF5 in 382 cases of nasopharyngeal carcinoma(NPC)and the EBV-positive cell line C666-1 cells were detected by real-time quantitative PCR. Livak(2-ΔΔCt)relative quantitative analysis and C666-1 as control were used to evaluate the copy numbers of EBV,where fold change ≥1.64 was regarded as high copy number group,otherwise as the low copy number group. Results The copy numbers of EBV DNA ranged from 0 to 34.78 with the average of 1.64. According to the univariate analysis,as compared to the low copy number group,the high copy number group had a higher risk of tumor progression(HR=2.06,95% CI:1.01-4.24)and distant migration(HR=3.06,95% CI:1.11-8.46);the differences were statistically significant(P0.5). Moreover,multivariate prognostic analysis revealed that high copy numbers of EBV was an independent predictor for PFS and DMFS. Conclusion High infective copy numbers of EBV exerts an important role in progression and distant metastasis of NPC patients. Monitoring the copy numbers of EBV DNA from NPC patients is helpful for early detection of disease progression and metastasis.
