摘要
目的比较Site-Rite 5超声探头颈内静脉压迫法和传统"指压法"对头颈部肿瘤患者经外周静脉置入中心静脉导管(PICC)置管的影响。方法采用回顾性研究2014年6月至12月行PICC置管的头颈部肿瘤患者470例设为对照组,采用传统"指压法"阻断颈内静脉;2016年1月至6月行PICC置管的头颈部肿瘤患者470例设为观察组,应用Site-Rite 5超声探头颈内静脉压迫法阻断颈内静脉。比较2组患者PICC一次置管成功率、颈内静脉异位率、置管时间、经济费用及患者满意度的差异。结果观察组在以上各方面均优于对照组(指压法组),2组比较差异具有统计学意义(P<0.05)。结论 Site-Rite 5超声探头颈内静脉压迫法在头颈部肿瘤患者PICC置管中可提高一次性置管成功率,降低异位颈内静脉发生率,缩短置管时间,减少不必要经济费用,提升患者满意度,值得推广。
Objective To compare the difference between internal jugular vein compression method guided by Site-Rite V ultrasonic probe and finger pressing method in peripherally inserted central catheter （PICC） for pa- tients with head and neck cancer. Methods Retrospective analysis method was used. 470 patients with head and neck cancer using finger pressing method in PICC from June 2014 to December 2014 were assigned to the control group, and 470 patients with head and neck cancer using internal jugular vein compression method guided by Site- Rite V ultrasonic in PICC from January 2016 to June 2016 were assigned to the observation group. We compared the differences in the success rate of once catheterization, the rate of ectopic jugular vein, the time of indwelling catheter, economic cost and patient satisfaction between the control group and the observation group. Results The differences in the success rate of one-time catheterization, the rate of ectopic jugular vein, the time of indwell- ing catheter, economic cost and patient satisfaction between the two groups was statistically significant （P〈 0. 05）. The above indexes of the observation group were better than those of the control group. Conclusion Inter- nal jugular vein compression method, guided by Site-Rite V ultrasonic probe in peripherally inserted central cathe- ter （PICC） for patients with head and neck cancer can improve the success rate of one-time catheterization, reduce the incidence of ectopic internal jugular vein, shorten the time of indwelling catheter, reduce unnecessary economic costs, and improve patient satisfaction. This method is worthy of being promoted.
