摘要
目的观察改良体位在PICC置管过程中的应用及对置管并发症的影响。方法选取本院2015年1月~2017年1月采用PICC行放化疗的患者90例,随机分为观察组及对照组,各45例。对照组取常规置管体位,观察组取改良体位,观察两组患者一次置管成功率、颈内静脉异位率及置管并发症发生情况。结果观察组置管用时少于对照组,差异具有统计学意义(P<0.05);观察组一次置管成功率(84.44%)高于对照组(57.78%),颈内静脉异位率(2.22%)低于对照组(17.78%),差异具有统计学意义(P<0.05);观察组机械性静脉炎、血栓、感染发生率分别为2.22%、2.22%、0%,低于对照组的20%、17.78%、13.33%,差异具有统计学意义(P<0.05)。结论 PICC置管患者置管过程中使用改良体位,患者一次置管成功率高,置管用时少,置管后颈内静脉异位发生少,置管并发症少,临床效果显著,应用价值高。
Objective To observe the application of improved posture in cathetering and its effect on complica- tions. Methods 90 patients with PICC chemoradiotherapy in our hospital from January 2015 to January 2017 were randomly divided into observation group and control group, 45 cases in each. Control group took rou- tine cathetering posture, observation group took improved posture. The success rate of one-time cathetering, the ectopic rate of internal carotid artery and complications of two groups were observed. Results The catheter- ing time of observation group was less than control group （P〈0.05）; The success rate of one-time cathetering of observation group （84.44%） was higher than control group （57.78%）, the ectopic rate of internal carotid artery of observation group （2.22%） was lower than control group （17.78%） （P〈0.05）; The incidence of mechanical phlebi- tis, thrombus, infection of observation group were 2.22%, 2.22%, 0%, which were lower than 20%, 17.78%, 13.33% of control group （P〈0.05）. Conclusion Improved posture in PICC cathetering has high success rate of one-time cathetering, less cathetering time, less ectopic rate of internal carotid artery and less cathetering compli- cations, which has significant clinical effect and high application value.
