摘要
目的探讨葡萄糖转运蛋白1(GLUT-1)和己糖激酶-Ⅱ(HK-Ⅱ)蛋白在乳腺癌组织中的表达及其预测患者预后的价值。方法选取2016年1月至2016年7月行手术治疗的乳腺癌患者32例为研究对象,术后随访至少半年。术后均采用免疫组织化学法行乳腺癌组织和癌旁组织GLUT-1和HK-Ⅱ蛋白表达的检测,比较不同预后患者的乳腺癌组织GLUT-1和HK-Ⅱ蛋白表达情况,分析其表达与预后的关系,进一步评价其预测患者预后的价值。结果32例患者随访期间的复发转移率为31.25%。乳腺癌组织GLUT-1蛋白阳性表达率(65.63%)及HK-Ⅱ蛋白阳性表达率(68.75%)明显高于其癌旁组织(9.38%,18.75%;P均<0.01);复发转移患者乳腺癌组织GLUT-1蛋白阳性表达率(100.00%)和HK-Ⅱ蛋白阳性表达率(100.00%)均高于无复发转移患者(50.00%,54.55%;P均<0.05)。Logistic回归分析结果显示,乳腺癌组织GLUT-1和HK-Ⅱ蛋白表达与其复发转移均密切相关(P均<0.01)。诊断性试验四格表分析结果显示,乳腺癌组织GLUT-1蛋白表达预测其复发转移的敏感度、特异度、准确性分别为47.62%、100.00%、65.63%,HK-Ⅱ蛋白表达分别为45.45%、100.00%、62.50%,两者联合预测分别为86.36%、100.00%、90.63%。结论乳腺癌组织GLUT-1和HK-Ⅱ蛋白表达升高且两者联合预测患者预后的价值良好,可能作为乳腺癌预后评估指标和临床治疗靶点。
Objective To investigate the expressions of glucose transporter 1 (GLUT-1) and hexokinaseⅡ(HK-Ⅱ ) in breast cancer tissues and the value of evaluating prognosis of breast cancer patients. Methods Thirty-two patients with breast cancer who underwent surgery treatment between January 2016 and July 2016 were selected as research objects. The patients were followed up for six months at least after operation. After operation,immunohistochemical method was used to detect the expressions of GLUT-1 and HK-Ⅱ proteins in breast cancer tissues and paracancerous tissues. The expression levels of GLUT-1 and HK- Ⅱproteins in breast cancer tissues of patients with different prognosis were compared, and the relationship between the expressions and prognosis was analyzed to further evaluate the value of predicting patients' prognosis. Results Recurrence and metastasis rate during follow-up was 31.25% in 32 patients. The positive expression rates of GLUT-1 protein(65.63% ) and HK- Ⅱ protein (68.75%) in breast cancer tissues were all significantly higher than those in paracancerous tissues (9. 38%, 18.75% ) ( all P 〈 0. 01 ). The positive expression rates of GLUT-1 protein ( 100. 00% ) and HK- Ⅱ protein ( 100.00% ) in breast cancer tissues of patients with recurrence and metastasis were all significantly higher than those in breast cancer tissues of patients without recurrence and metastasis (50.00% ,54.55% ) ( all P 〈 0. 05 ). Logistic regression analysis showed that GLUT-1 and HK- Ⅱ expressions were closely correlated with recurrence and metastasis of tumor (all P 〈 0.01 ). Diagnostic test of four case analysis showed that sensitivity, specificity and accuracy of predicting recurrence and metastasis were 47.62% , 100.00% ,65.63% for GLUT-1 expression and45.45%, 100.00% ,62.50% for HK- Ⅱexpression and 86.36%, 100.00% ,90.63% for the combination both of them.
