摘要
目的提高延续护理效果,满足患者健康管理需求。方法根据专家咨询患者需求确定延续护理信息系统功能及基本架构,基于医院信息系统(HIS)设计系统模块,延续护理信息系统经调试后在妇产科进行4个月的试运行,对318例患者实施院内外健康教育、出院随访、医患互动、跟踪干预、问卷测评等延伸服务。结果试运行期间,经延续护理信息系统完成健康咨询208例次、提醒用药752例次、推送消息733例次、进行健康干预72例次、发布健康讲座12讲。受访的医护人员及患者或家属85.0%以上认可该系统的便捷性、获益性、稳定性、安全性等。结论基于HIS系统的延续护理信息平台构建合理,基本能满足签约患者延续护理服务需求,有利于出院患者持续有效的健康管理。
Objective To improve the effect of transitional nursing service and to meet patients＇ need for health management. Methods The functions and structure of the transitional nursing information platform were established by using Delphi Technique. Then the modules of the platform were designed based on HIS and underwent revisions during the first trial period. After that, the platform was applied to 318 patients of obstetrics and gynecology department for further pilot run for transitional care, which included health education, after-discharge follow-ups, doctor-nurse patient interations, tracking and intervening, questionnaire surveys, etc. Re- sults During pilot run, medical staffers accomplished a tally of 208 health consults, 752 counts of medication remindering, 733 push messages, 72 counts of health intervention, and issued the information of 12 lectures, through the transitional nursing information platform. More than 85.0% of the medical staffers and the patients acknowledged the convenience, benefits, stability, safety,etc. , of the system. Conclusion The transitional nursing information platform based on HIS has an agreeable structure, and is able to grossly satisfy the needs of transitional care among patients who register for the service, and maintain health management for dis- charge patients.
