血清Hcy、Cys C、HbA1c及尿β2-MG水平的变化对糖尿病肾病的诊断价值 被引量:8

Diagnostic value of serum Hcy, Cys C, HbA1c and urine β2-MG level changes on diabetic nephropathy

摘要 目的 探讨血清同型半氨酸(Hcy)、胱抑素C(Cys C)、糖化血红蛋白(HbA1c)及尿β2-微球蛋白(β2-MG)水平的变化对糖尿病肾病的诊断价值.方法 选取2015年2月至2016年3月82例糖尿病患者为研究组,另选取27例同期体检健康者对照组.检测对比两组血清Hcy、Cys C、HbA1c、尿β2-MG水平及其随病情进展变化情况,分析各指标与24 h尿蛋白排泄率(UAER)的相关性.结果 研究组Hcy、Cys C、HbA1c及β2-MG水平均明显高于对照组,差异有统计学意义(P〈0.05).单纯糖尿病组、轻度糖尿病肾病组、中重度糖尿病肾病组Hcy、Cys C、HbA1c、β2-MG、UAER水平与对照组比较差异有统计学意义(P〈0.05);且随着病情加重,各指标水平不断增高,差异有统计学意义(P〈0.05).血清Hcy、Cys C、HbA1c、β2-MG水平与UAER均呈正相关(P〈0.05).结论 可将血清Hcy、Cys C、HbA1c及尿β2-MG作为糖尿病肾病早期诊断的重要指标,其可反映肾功能损伤程度,具有推广价值. Objective To investigate the diagnostic values of serum levels of Hcy, Cys C, HbA1c and urine β2-MG on diabetic nephropathy.Methods Eighty-two patients with diabetes mellitus who received the research criteria from February 2015 to March 2016 were selected as study group, and 27 healthy persons were selected as the control group.The serum Hcy, Cys C, HbA1c, urine β2-MG levels and the progress of the disease changes were observed, and the correlation between targets and UAER were analyzed.Results The levels of Hcy, Cys C and HbA1c, β2-MG in the study group were significantly higher than those in the control group (P〈0.05).There were significant differences in the levels of Hcy, Cys C, HbA1c, β2-MG, UAER among the simple diabetic group, mild diabetic nephropathy group, severe diabetes nephropathy group and the control group(P〈0.05).With the severity of the disease increased, the levels of indicators were rising, the differences were significant (P〈0.05).Serum Hcy, CysC, HbA1c, β2-MG levels were positively correlated with UAER (P〈0.05).Conclusions Serum Hcy, Cys C, HbA1c and urine β2-MG can be used as important indexes for the early diagnosis of diabetic nephropathy, which also can reflect the degree of renal injury, and has the value of popularization.
