摘要
目的探讨血小板捐献者HLA-A、B和HPA1-17系统基因的分布特点,建立临沂地区已知HLA、HPA基因型机采血小板供者库。方法对临沂地区205位无血缘关系的血小板捐献者分别采用PCR-SSOP、PCR-SSP方法对HLA-A、B位点、HPA1-17系统进行基因分型,计算基因频率和基因型频率。结果检出HLA-A特异性抗原位点17个,频率较高抗原的有A2(0.2665)、A30(0.1418)、A24(0.1394)、A11(0.1149);检出HLA-B特异性抗原位点31个,频率较高的有B13(0.1663)、B62(00758)、B61(0.0636)、B58(0.0661)。HPA1-17中,HPA7-14、16、17呈单特异性,未检出相应的等位基因HPAb,杂合度最高的是HPA15,基因型HPA15a/15a、HPA15a/15b、HPA15b/15b,频率分别是0.3170,0.5073,0.1756,其次为HPA3,基因型HPA3a/3a、HPA3a/3b、HPA3b/3b,频率分别是0.3516,0.4293,0.2146,不配合率较高的是HPA3和HPA15,均为36.99%。频率最高的HPA基因组合为HPA(1,4,5,6,7-14,16,17)aa-2aa-3ab-15ab(0.1854)。结论探查临沂地区血小板捐献者HLA-A、HLA-B和HPA1-17系统区域分布特点,可为PTR患者寻找匹配的血小板供者、建立全省联网的血小板捐献者已知型HLA、HPA分型库提供资料。
Objective To study the distribution characteristics of HLA-A, HLA-B and HPAl-17 genes of platelet donors and to set up the classified platelet banks of HLA and HPA genes in Linyi City. Methods 205 platelet donors of no genetic connection from Linyi City underwent genotyping of HLA-A, HLA-B and HPAl-17 by through PCR-SSOP and PCR-SSP respectively. The gene and genotypic frequencies were calculated. Results 17 HLA-A specific antigen sites were discovered and A2, A30, A 24 and All showed higher frequency than others. 31 HLA- B specific antigen sites were discovered and B13, B62, B61 and B58 showed higher frequency than others. HPAb was not discovered from HPA1 to HPA17, but monomorphic HPA7-14,HPA 16 and HPA 17 were found. HPA15 exhibited the highest heterozygosity, with the gene types being HPA15a/15a, HPA15a/15b and HPA15b/15b. The gene types of HPA3 were HPA3a/ 3a, HPA3a/3b and HPA3b/3b. Both HPA3 and HPA15 showed lower matching rate of 36.99%. The most common HPA genotype combination was HPA（1, 4,5,6,7-14,16,17） aa-2aa-3ab-15ab （ 0. 1854 ）. Conelusion Studying the distribution characteristics of HLA-A, HLA-B and HPA1-17 genes of platelet donors in Linyi City can help to find the matching platelet donors for PTR patients and to provide some research ma- terials for Shandong Provincial HLA and HPA genotyping banks.
