摘要
目的:探讨常规野放射治疗及调强放射治疗对人乳头瘤病毒(HPV)不同亚型感染宫颈癌患者的临床效果,为临床采用有效的放疗法治疗宫颈癌提供依据。方法:选取2008年1月至2010年10月在医院进行治疗的宫颈癌患者,共102例,将患者随机分为对照组及试验组。对照组患者进行常规野放射治疗,试验组患者进行调强放射治疗。结果:对照组患者经过放疗后有效者为34例(68.0%),试验组患者经放疗后有效者47例(90.4%),试验组有效率显著高于对照组(P<0.05)。试验组患者中,缓解者47例,缓解得以维持的时间为2~5年,平均4.5年,治疗后19例患者肿瘤出现新的进展(1例为治疗后处于稳定的患者);对照组患者中34例缓解,缓解得以维持的时间为1.5~5年,平均3.8年,治疗后24例患者肿瘤出现新的进展(4例为治疗后处于稳定的患者)。对照组患者经治疗后存活时间达5年及以上的为34例(68.0%),试验组患者经治疗后存活时间达5年及以上的为44例(84.62%),试验组患者中存活时间超过2年及5年的比率均显著高于对照组(P<0.05)。结论:常规野放射治疗及调强放射治疗对HPV感染的宫颈癌患者均具有临床效果,但调强放射治疗效果更显著,存活时间更长。
Objectives： To investigate the clinical effect of conventional field radiotherapy and intensity modulated radiation therapy （IMRT） for human papillomavirus （HPV） infection in cervical cancer patients with dif- ferent subtypes, to provide basis for the effective treatment of cervical cancer in clinical. Methods： 102 patients with cervical cancer in our hospital from January 2008 to October 2010 were selected and randomly divided into con- trol and experimental group. The control group received routine field radiation therapy, while experimental group re- ceived IMRT. Results： The response rate of control group and experimental group was 34 （ 68.0% ） and 47 （90. 4% ） respectively, with statistically significant difference （P 〈 0. 05 ）. Of the 47 patients effective in the ex- perimental group, the remission lasted 2 to 5 years, with an average of 4.5 years, and 19 ones with new progress （ 1 patient stable）. Of the 34 patients effective in the control group, the remission lasted I. 5 to 5 years, with an aver- age of 3.8 years, and 24 patients with new progress （4 patients stable）. 34 patients （68.0%） and 44 patients （84.62%） survived for more than 5 years after treatment in the control group and experimental group respectively, with statistically significant difference （ P 〈 0. 05 ）. Conclusion： The conventional field radiotherapy and IMRT are both effective for cervical cancer patients with HPV infection, but IMRT is more significant with longer survival time.
