摘要
目的:探讨血清透明质酸(HA)、层粘蛋白(LN)、Ⅳ型胶原(Ⅳ-C)及人Ⅲ型前胶原肽(PⅢNP)联合丙氨酸转氨酶(ALT),天门冬氨酸转氨酶(AST)、血清白蛋白(ALB)、总胆红素(SB)在慢性乙型肝炎诊断中的价值及其相关性。方法:联合检测健康体检者50例及慢性乙型肝炎感染者162例血清中肝纤维化4项血清学指标HA、LN、CⅣ、PⅢNP及肝功能指标ALT、AST、ALB、SB的含量。计量资料总体均数差异的比较采用kruskal-wallis H检验,各组间两两比较采用Nemenyi法检验。结果:与健康组相比,血清肝纤维化指标HA、CⅣ和PⅢNP,血清肝功能指标中ALT和AST在慢性肝炎变化的各个时期均有统计学差异(P<0.05);血清CⅣ在中、重度肝炎及肝纤维化组中与肝功能指标有相关性(P<0.05),血清PⅢNP在重度肝炎组与SB有相关性(P<0.05)。结论:血清肝纤维化指标HA、CⅣ和PⅢNP在轻度肝炎时即有显著升高,并且与慢性肝炎的病变程度呈正相关,CⅣ和PⅢNP与肝功能指标存在相关性,联合血清肝功能指标将更有利于准确评价慢性乙型肝炎患者病变程度。
Objective:To investigate the diagnostic value and relationships among hyaluronic acid(HA),laminin(LN),typeⅣcollagen(CⅣ),human typeⅢcollagen peptide(PⅢNP)and alanine aminotransferase(ALT),aspartate aminotransferase(AST),serum albumin(ALB),total bilirubin(SB)in chronic hepatitis B.Methods:Hepatic Fibrosis Indexes including HA,LN,CⅣand PⅢNP and Liver Function Indexes including ALT,AST,ALB and SB in the serum of 50healthy subjects and 162patients with chronic hepatitis B had been examined.The Kruskal-Wallis H test had been used to compare the differences of the data overall means,the Nemenyi test for comparison between any two groups.Results:There were statistically significant differences in HA,CⅣand PⅢNP in the hepatic fibrosis indexes and ALT and AST in the liver function indexes in serum between the healthy subjects and the patients(P<0.05).CⅣhad correlation with liver function indexes in both the moderate and the severe hepatitis groups and the cirrhosis group(P<0.05).PⅢNP had correlation with SB in the severe hepatitis group(P<0.05).Conclusion:The levels of HA,CⅣand PⅢNP in serum have significantly increased in mild hepatitis,and have positive correlation with the degrees of chronic hepatitis.CⅣand PⅢNP have correlation with liver function indexes in serum.Combination of the hepatic fibrosis indexes with the liver function indexes in serum is helpful for accurate assessment of the degrees of chronic hepatitis B.
