摘要
目的探讨非酒精性脂肪肝(NAFLD)患者血清细胞角蛋白18(CK-18)、内脂素的变化及与肝纤维化的关系。方法选取2015年6月至2018年5月确诊的NAFLD患者66例作为NAFLD组、选取同期健康体检对象66例作为对照组。检测2组的血清CK-18、内脂素、三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FPG)、胰岛素抵抗指数(HOMA-IR)、Ⅲ型胶原前肽(PCⅢ)、Ⅳ型胶原(PCⅣ)、透明质酸(HA)并进行比较分析。结果 NAFLD组患者的血清CK-18水平显著的高于对照组,差异有统计学意义(P<0.05);NAFLD组患者的内脂素水平显著的低于对照组,差异有统计学意义(P<0.05);NAFLD组患者的血清PCⅢ、PCⅣ、HA水平高于对照组,差异有统计学意义(P<0.05);NAFLD组患者的血清PCⅢ、PCⅣ、HA水平与血清CK-18均呈显著的正相关关系(P<0.05),NAFLD组患者的血清PCⅢ、PCⅣ、HA水平与血清内脂素均呈显著的负相关关系(P<0.05)。结论 NAFLD患者血清CK-18、内脂素水平显著升高,并且与患者纤维化指标水平升高关系密切。
Objective To investigate the correlation between the serum levels of cytokeratin 18(CK-18), visfatin and liver fibrosis indexes in patients with nonalcoholic fatty liver disease(NAFLD). Methods Sixty-six patients with NAFLD who were diagnosed in our hospital from June 2015 to May 2018 were enrolled as NAFLD group, and 66 healthy subjects who underwent physical examination in our hospital at the same period were enrolled as control group. The serum levels of CK-18, visfatin and triacylglycerol(TG), total cholesterol(TC), high-density lipoprotein cholesterol(HDL-C), low-density lipoprotein cholesterol(LDL-C), fasting blood glucose(FPG), insulin resistance index(HOMA-IR), type Ⅲ collagen propeptide(PCⅢ), type Ⅳ collagen(PCⅣ), hyaluronic acid(HA) were observed and compared between the two groups. Results The serum levels of CK-18 in NAFLD group were significantly higher than those in control group(P<0.05),however,the visfatin levels in NAFLD group were significantly lower than those in control group(P<0.05). Moreover the serum levels of PCⅢ, PCⅣ and HA in NAFLD group were significantly higher than those in control group(P<0.05). In addition the serum levels of PCⅢ, PCⅣ, HA were closely positively correlated with those of CK-18 in NAFLD group(P<0.05),however,which were closely negatively correlated with those of visfatin in NAFLD group(P<0.05).Conclusion The serum levels of CK-18 and visfatin in patients with NAFLD are significantly increased,moreover,which are closely related to the elevated levels of fibrosis indexes in patients with NAFLD.
