摘要
目的探讨睡眠呼吸暂停低通气综合征患者T淋巴细胞CD69表达的临床意义。方法选取2014年5月至2016年2月本院接收的符合标准的53例睡眠呼吸暂停低通气综合征患者为观察组,另外选择20例正常体检者作为对照组。根据多导睡眠监测仪检查结果将观察组患者分为轻度组(低通气指数5~20次/小时)、中度组(低通气指数21~40次/小时)和重度组(低通气指数>40次/小时)。比较患者的一般资料(年龄、体重指数、呼吸紊乱指数、血氧饱和度),分析观察组患者CD69表达率和可溶性CD40配体水平。结果观察组(轻度组、中度组、重度组)患者的年龄、体重指数、呼吸紊乱指数、血氧饱和度与对照组比较差异均有显著性(P<0.05),观察组患者CD69表达率和可溶性CD40配体水平明显高于对照组(P<0.05),重度组及中度组患者的CD69表达率和可溶性CD40配体水平明显高于对照组及轻度组(P<0.05);采用CAPA呼吸机治疗后,观察组CD69/CD3、可溶性CD40配体、最低血氧饱和度水平显著改善(P<0.05)。结论睡眠呼吸暂停低通气综合征患者T淋巴细胞CD69的表达率升高,且随病情加重而上升。T淋巴细胞CD69的表达率可能与睡眠呼吸暂停低通气综合征的发生发展有关。
Objective To investigate the clinical-significance of the expression of CD69 in T cells in patients with obstructive sleep apnea hypopnea syndrome. Method The observation group of 53 patients with sleep apnea hypopnea syndrome received in our hospital from May 2014 to February 2016 were selected; Another 20 normal subjects were selected as normal control group. According to the results of the multi-sleep monitor, the patients in the observation group were divided into mild group （AHI 5~20times/h）, moderate group （AHI 21 ~40 times/h） and severe group （AHI 〉40 times/h）. The two groups were compared with general information （age, body mass index, respiratory disturbance index, blood oxygen saturation）, and the expression rate of sCD40L and CD69 in the observation group were analyzed. Result The observation group （mild, moderate and severe） were age, body mass index, respiratory disturbance index, oxygen saturation compared with the control group with significant difference （P〈0.05）. The patients in the observation group the expression rate of CD69 and sCD40L was significantly higher than control group （P〈0.05）, severe and moderate sleep apnea hypopnea syndrome patients the expression rate of CD69 and sCD40L was significantly higher than control group and mild group （P〈0.05）; using CAPA ventilator after treatment, the observation group CD69/CD3,sCD40L and LsaO2 were significantly improved （P〈O.05）. Conclusion The expression rate of CD69 in T cells was increased in the patients with obstructive sleep apnea hypopnea syndrome, and increased with the aggravation of the disease, the expression rate of CD69 in T cells may be related to the occurrence and development of obstructive sleep apnea hypopnea syndrome.
