摘要
目的分析不同睡眠体位对阻塞性睡眠暂停低通气综合征(OSAHS)患者血氧饱和度和呼吸暂停低通气指数(AHI)的影响。方法选取OSAHS患者168例为研究对象,根据AHI分为正常组52例,轻中度组51例和重度组65例,多导睡眠图检测,分析不同体位下血氧饱和度下降指数(ODI)和AHI变化。结果正常组患者仰卧位AHI均显著高于左侧卧位和右侧卧位(P<0.05),轻中度患者仰卧位AHI显著高于右侧卧位(P<0.05),重度组不同体位AHI差异无显著性(P>0.05),不同组别左侧卧位和右侧卧位AHI差异无显著性(P>0.05)。正常组患者仰卧位ODI显著高于左侧卧位和右侧卧位(P<0.05),轻中度患者仰卧位ODI显著高于右侧卧位(P<0.05),重度组不同体位AHI差异无显著性(P>0.05)。正常组患者不同体位下AHI和ODI变化无显著差异,轻中度组AHI和ODI呈现明显下降趋势。重度组患者仰卧位和左侧卧位基本一致,均高于右侧卧位。结论不同程度OSAHS患者仰卧位AHI和ODI均高于侧卧位,提示睡眠体位疗法能够改善OSAHS症状。
Objective To assess the influence of body position on AHI and ODI during sleep in patients with obstructive sleep apnea hypopnea syndrome （OSAHS） with different severity. Methods 168 patients with OSAHS in this hospital were selected for this study, they were divided into normal group （52 cases） , mild to moderate group （51 cases） and severe group （65 cases） according to AHI, ODI and AHI in patients of these three groups at different positions, and they were compared by polysomnography. Results Normal AHI at supine position was significantly higher than that at right and left lateral decubitus （ P 〈 0 . 0 5 ） , AHI at mild to moderate supine position was significantly higher than that at right lateral position （ P 〈 0. 05） , the difference in AHI of sveere group at different position was not significant （ P 〉 0 . 0 5 ） , and AHI in different groups of the left and right lateral decubitus was not significant （ P 〉 0 . 0 5 ） . ODI at normal supine position was significantly higher than that at left and right lateral decubitus （ P 〈0.05）, ODI at mild to moderate supine position was significantly higher than that at right lateral position （P 〈0. 05） , AHI in severe group was not significantly different at various postures （ P 〉 0 . 05）. The difference in change of AHI and ODI in normal patients at different positions was not significant, AHI and ODI in mild to moderate groups showed a clear downward trend. AHI and ODI in severe patients at supine and left lateral decubitus were consistent, and they were higher than those at right lateral position. Conclusion AHI and ODI in patients with various degree of OSAHS at supine position are higher than those of patients at lateral position, thus it suggests that sleeping position therapy can improve the symptoms of patients with OSAHS.
