间歇性夹管训练对短期留置导尿患者膀胱尿道功能影响的Meta分析 被引量:12

Effects of Intermittent Clamping on Bladder and Urethra Function in Patients with Short-term Indwelling Urethral Catheters:A Meta-analysis

摘要 目的综合评价间歇性夹管训练对短期留置导尿患者膀胱尿道功能的影响。方法计算机检索美国国家生物技术信息中心(Public Medline,PubMed)、中国期刊全文数据库(China National Knowledge Infrastructure,CNKI)、中国生物医学文献数据库(Database of Chinese Biomedical Literature,CBM)等数据库中近10年关于短期留置导尿患者采取间歇性夹管训练与持续开放引流对比的随机对照试验(randomized controlled trials,RCT),评价纳入研究质量、提取数据、采用RevMan 5.3统计软件进行Meta分析。结果共纳入12个研究。Meta分析结果显示,两组患者在排尿成功情况[RR=1.00,95%CI(0.97,1.03),P=0.79]、残尿量异常情况[RR=0.96,95%CI(0.55,1.67),P=0.89]、尿潴留[RR=1.02,95%CI(0.87,1.21),P=0.78]、再置管[RR=1.02,95%CI(0.86,1.22),P=0.80]、尿路感染[RR=1.08,95%CI(0.75,1.57),P=0.67]等的方面差异均无统计学意义(均P>0.05)。结论建议短期留置导尿患者留置导尿期间不采取间歇性夹管训练。 Objective To analyze the effects of intermittent clamping on bladder and urethra function in patients with short-term indwelling urethral catheters.Methods A systematic literature searching method for studies which were published on PubMed,CNKI and CBM was performed in last ten years.We included randomized controlled trials(RCT)related to intermittent clamping and free drainage in patients with short-term indwelling urethral catheters.We assessed the methodology quality of included researches,and extracted data.RevMan 5.3was used for meta-analysis.Results Total of 12 studies were included.Meta-analysis showed that there was no statistical difference between the two groups in urination success[RR=1.00,95%CI(0.97,1.03),P=0.79],abnormal residual urine volume[RR=0.96,95%CI(0.55,1.67),P=0.89],urinary retention [RR=1.02,95%CI(0.87,1.21),P=0.78]:reinsertion of indwelling urethral catheter [RR =1.02,95%CI(0.86,1.22),P =0.80]:and urinary infection[RR=1.08,95%CI(0.75,1.57),P=0.67](all P〉0.05).Conclusions We suggest that intermittent clamping is not necessary for patients with short-term indwelling urethral catheters.
