摘要
神经电刺激和电调节技术已经广泛和成功地应用于下尿路功能障碍患者,其中对骶神经根进行电刺激是应用最广泛的一种电刺激方式。骶神经前根电刺激(SARS)配合完全性骶神经后根切除术,经临床验证可有效地提高脊髓损伤(SCI)患者的生活质量。骶神经电调节(SNM)无需切除神经即可调节难治性膀胱功能障碍,其具体机制尚未完全阐明,可能是多种机制共同作用的结果,其中包括刺激骶神经时通过传入和/或传出神经抑制了逼尿肌过度活动。综述了目前国内外SARS和SNM技术的最新发展,以期为该领域的研究者提供有益借鉴。
Electrical stimulation of nerve has been successfully applied to treating patients with lower urinary tract dysfunctions.Stimulation at the level of the sacral nerve roots is the most widely applied technique of neurostimulation.Sacral anterior root stimulation(SARS) with sacral deafferentation has been shown to be very effective in significantly improving the quality of life for many patients with spinal cord injury(SCI).Instead of the deafferentation,sacral neuromodulation(SNM) improves refractory bladder dysfunction in patients by a working mechanism not fully understood,probably a combination of different suggested theories,involving detrusor inhibition via stimulation afferent and/or efferent sacral nerve fibers.This article provides a comprehensive review of the current status in SARS and SNM for bladder function rehabilitation.
