摘要
目的探究盆底肌瑜伽联合间歇性夹管训练在宫颈癌根治术后膀胱功能障碍患者康复中的应用效果.方法选择2020年1月—2022年3月于滕州市妇幼保健院行宫颈癌根治术治疗后发生膀胱功能障碍的患者96例为研究对象,按随机原则分为研究组(n=48)及对照组(n=48).对照组术后进行常规康复训练,研究组术后进行盆底肌瑜伽联合间歇性夹管训练,均持续干预至术后4周.对比两组患者的尿流动力学指标及术后并发症发生率.结果干预后,研究组的最大尿流时逼尿肌压力、最大膀胱容量、最大尿流率及膀胱初感容量均高于对照组,膀胱残余尿量低于对照组,组间差异有统计学意义(P<0.05).研究组的术后膀胱刺激征、尿失禁及尿潴留发生率分别为4.17%、2.08%、10.42%,均低于对照组,组间差异有统计学意义(P<0.05).结论盆底肌瑜伽联合间歇性夹管训练能更好地促进宫颈癌根治术后伴膀胱功能障碍患者的膀胱功能恢复,降低尿失禁等并发症的发生风险,可于临床推广.
Objective To explore the application effect of pelvic floor muscle yoga combined with intermittent tube clamping training in the rehabilitation of patients with bladder dysfunction after radical surgery for cervical cancer.Methods A total of 96 patients with bladder dysfunction after radical resection of cervical cancer in Tengzhou Maternal and Child Health Hospital from January 2020 to March 2022 were selected as the research objects and randomly divided into a study group(n=48)and a control group(n=48).The control group received routine rehabilitation training after operation,while the study group received pelvic floor muscle yoga combined with intermittent tube clamping training after operation.Intervention was continued until 4 weeks after operation in both groups.The urodynamic parameters and the incidence of postoperative complications were compared between the two groups.Results After intervention,the detrusor pressure,maximum bladder volume,maximum urinary flow rate and initial bladder sensing volume of the study group were higher than those of the control group,and the bladder residual urine volume was lower than that of the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of bladder irritation,urinary incontinence and urinary retention in the study group were 4.17%,2.08%and 10.42%,respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Pelvic floor muscle yoga combined with intermittent tube clamping training can better promote the recovery of bladder function in patients with bladder dysfunction after radical resection of cervical cancer,reduce the risk of incontinence and other complications,which can be popularized in clinical practice.
