摘要
目的:系统评价头针治疗中风后抑郁症(post-stroke depression,PSD)的临床疗效。方法:通过计算机和手工检索关于头针治疗中风后抑郁症的临床文献,采用循征医学的评价方法,对符合要求的随机对照试验进行收集,共纳入13篇文献,总计1198例中风后抑郁症患者,采用Revman5.3软件进行Meta分析。结果:Meta分析提示头针治疗中风后抑郁的总有效率优于其他治疗方法,其差异有统计学意义[总有效率RD=0.09,95%CI(0.05,0.14),P<0.001]。在HAMD量表评分的系统分析中,仅2项研究的Meta分析结果提示体针组PSD患者的量表评分高于头针组[WMD=-3.72,95%CI(-4.84,-2.63),P<0.00001],差异有统计学意义。结论:头针治疗中风后抑郁症有效,但入选的随机对照试验质量不高,降低了这一结论的可靠性,尚需高质量、大样本、多中心的随机研究进一步验证。
Objective：The clinical efficacy of scalp acupuncture for post stroke depression was systematically reviewed. Methods：The randomized control trials（RCTs）regarding scalp acupuncture for post stroke depression were performed with cochrance system assessment methods by computerized and manual retrieval. Meta-analysis was conducted with the assistance of Revman5.3 software. Results：13 RCTs involving 1198 cases of post-stroke depression were included. Meta-analysis indicated that the total effective rate of scalp acupuncture group were significantly superior to those of control group [RD=0.09,95%CI（0.05,0.14）,P〈0.001]. In systematic analysis of HAMD score,only 2 RCTs indicated that score of body acupuncture group was statistically higher than that of scalp acupuncture group [WMD=-3.72,95%CI（-4.84,-2.63）,P〈0.00001]. Conclusion：The results of our review suggest that scalp acupuncture may be effective for PSD,but the poor quality of these included trials precludes us from drawing the firm conclusions. High-quality,large-sample and multi-center RCTs are needed to perform further verification.
