摘要
目的观察胫骨平台骨折行锁定加压钢板内固定术治疗后再辅以壮筋续骨汤治疗的临床效果。方法 88例符合入组标准的胫骨平台骨折患者,随机分为研究组和对照组,每组44例。对照组单纯行锁定加压钢板内固定术治疗;研究组在对照组的基础上,于术后口服壮筋续骨汤治疗。治疗结束后采用Rasmussen膝关节功能评定标准评价膝关节恢复情况,比较两组患者平均住院时间、骨折愈合时间及最早负重时间,记录手术并发症发生情况。结果研究组膝关节功能恢复情况明显优于对照组,差异具有统计学意义(P<0.05)。研究组平均住院时间为(6.25±1.14)d、骨折愈合时间为(3.0±1.0)个月、最早负重时间为(2.3±1.2)个月,均短于对照组的(9.38±1.22)d、(4.6±0.8)个月、(4.4±1.3)个月,差异均具有统计学意义(P<0.05);研究组术后并发症发生率为4.55%,低于对照组的18.18%,差异具有统计学意义(P<0.05)。结论壮筋续骨汤联合锁定加压钢板内固定治疗胫骨平台骨折疗效显著,患者术后骨折愈合快、膝关节功能恢复好、术后并发症少,且能更早开始部分负重,改善患者生活质量。
Objective To observe the clinical effect of Zhuangjin Xugu decoction combined with locking compression plate internal fixation for the treatment of tibial plateau fracture. Methods A total of 88 eligible patients with tibial plateau fracture were randomly divided into research group and control group, with 44 cases in each group. The control group received locking compression plate internal fixation only, and the research group received oral Zhuangjin Xugu decoction for treatment on the basis of the control group. Their knee joint recovery situation was evaluated by Rasmussen knee joint function assessment standard in off-therapy, and comparison were made on average hospital stay time, fracture healing time, earliest weight bearing time and occurrence of complications in operation between two groups. Results The research group had better knee joint recovery situation than that in the control group, and the difference had statistical significance（P〈0.05）. The research group had average hospital stay time as（6.25±1.14） d, fracture healing time as（3.0±1.0） months and earliest weight bearing time as（2.3±1.2） months, which were all shorter than（9.38±1.22） d,（4.6±0.8） months and（4.4±1.3） months in the control group, and their differences had statistical significance（P〈0.05）. The research group had lower incidence of postoperative complications as 4.55% than 18.18% in the control group, and the difference had statistical significance（P〈0.05）. Conclusion Zhuangjin Xugu decoction combined with locking compression plate internal fixation shows obvious curative effect for the treatment of tibial plateau fracture, with quick postoperative fracture healing, good recovery of knee joint function and few postoperative complications in patients, as well as early partial weight bearing and improvement of quality of life in patients.
