摘要
目的探讨老年男性2型糖尿病患者骨密度与炎症因子的相关性。方法选取我院老年病科住院的280例老年男性2型糖尿病患者,计算体质指数(BMI),测定空腹血糖(FBG)、糖化血红蛋白(HBA1c)、C反应蛋白(CRP)、单核细胞趋化因子(MCP-1)、肿瘤坏死因子(TNF-α)、白细胞介素6(IL-6)及骨密度参数(T值和Z值),分析骨密度与炎症因子的相关性。结果糖尿病患者随着骨密度降低,年龄增加(F=8.928,P<0.01)、HBA1c增加(F=7.290,P<0.01),炎症因子CRP、TNF-α及IL-6均增高(F分别为6.928、8.301、4.290,均P<0.05);BMD与年龄、CRP、TNF-α、IL-6呈负相关(r值分别为-0.229、-0.079、-0.090、-0.089,均P<0.05),与BMI呈正相关(r=0.088,P<0.05),通过Logistic回归分析校正混杂因素后显示,年龄为骨质疏松的独立危险因素(OR值分别为1.07,P<0.05)。结论老年男性2型糖尿病患者骨密度与炎症因子存在相关性,炎症因子在DOP的发生发展过程中起到重要作用,但并非其独立危险因素。
Objective To evaluate the relationship between bone mineral density and inflammatory cytokines in elderly male patients with type 2 diabetes.Methods In total 280 elderly male patients with type 2 diabetes were selected for health examination.Body mass index(BMI) were calculated.Levels of FBG,HBA1 c,CRP,MCP4,TNF-α,IL-6 and Bone mineral density parameters(T and Z) were assessed.The correlations of BMD with inflammatory cytokines were analyzed.Results In patients with type 2 diabetes,with the decrease of BMD,age,HBA1 c CRP,TNF-α,IL-6 were increased(F = 8.928,7.290,6.928,8.301,4.290,all P〈0.01),Spearman analysis showed that BMD was negatively correlated with age,CRP,TNF-α and IL-6(r =-0.229,-0.079,-0.090,-0.089,all P〈0.05),and was positively correlated with BMI(r =0.088,P〈0.05).Age was an independent risk factor for diabetic osteoporosis(DOP)(OR = 1.07,P 〈0.05).Conclusions The present study demonstrated a relationship between BMD and inflammatory cytokines in elderly patients with type 2 diabetes.The inflammatory cytokines had important effect on the development of DOP,but were not independent risk factors.
