摘要 目的:探讨非糖尿病慢性肾衰竭合并胰岛素抵抗患者的动态血糖波动特点及临床意义。方法:63例符合入选标准且同意参加试验的受试者分为胰岛素抵抗组(IR组,34例)和非胰岛素抵抗组(非IR组,29例),行72 h动态血糖监测,计算并比较两组24 h平均血糖水平(24 h mean blood glucose,24 h MBG)及其标准差(standard deviation,SDBG),日内平均血糖波动幅度(mean amplitude of glycemic excursions,MAGE),最大血糖波动幅度(large amplitude of glycemic excursions,LAGE)以及空腹血糖变异系数(fasting plasma glucose coefficient variation,FPG-CV)。结果:IR组的日内平均血糖波动幅度(MAGE)、最大血糖波动幅度(LAGE)及空腹血糖变异系数(FPG-CV)均明显高于非IR组(P<0.05),凌晨2点血糖明显升高。结论:合并IR的非糖尿病慢性肾脏病患者存在血糖异常波动,且夜间血糖升高,临床需要关注。 Objective: To investigate the blood sugar fluctuations and its clinical significance in non- diabetic patients with chronic renal failure( CRF) and insulin resistance. Methods: A total of 63 patients were enrolled in this study. They all met the inclusion and exclusion criteria and signed informed consent forms before enrollment. Patients were divided into the insulin resistance group( IR group,n = 34) and the control group( non- IR group,n = 29). 72- hours continuous glucose monitoring( CGM) was applied on all the subjects. 24Hmbg( 24 h mean blood glucose),SDBG( standard deviation),MAGE( mean amplitude of glycemic excursions),LAGE( large amplitude of glycemic excursions,LAGE) and FPG- CV( fasting plasma glucose coefficient variation)were calculated according to the results of CGMS( continuous monitoring system). Results: The mean amplitude of glycemic excursions( MAGE),the large amplitude of glycemic excursions( LAGE),the fasting plasma glucose coefficient variation( FPG- CV)and the blood glucose level on 2a. m. were significantly different between the two groups( P〈0. 05). Conclusion: Non- diabetic CRF patients with insulin resistance prone to abnormal blood sugar fluctuations and higher blood glucose level on 2am.
