摘要
目的探讨影响骨肉瘤预后的相关因素。方法选择2008年1月—2015年7月安康市中心医院收治并确诊为骨肉瘤95例作为研究对象,提取性别、年龄、肿瘤部位、手术方式及营养水平等多项可能影响预后的因素,采用Kaplan Meier法计算患者总体生存率,应用Log Rank检验进行预后影响因素的单因素分析,应用多元逐步Cox回归模型进行预后影响因素的多因素分析。结果 95例骨肉瘤随访13~90个月,平均41.3个月;随访期间67例死亡,28例存活,中位生存期为39个月;3年总体生存率为52.6%,5年总体生存率为33.7%。Log Rank检验单因素分析结果显示,骨肉瘤患者预后与肿瘤部位、营养水平、碱性磷酸酶水平、肿瘤分期、病理性骨折密切相关(P<0.05)。多元逐步Cox回归分析结果显示,肿瘤部位和病理性骨折是影响骨肉瘤患者预后的独立危险因素,营养水平则是影响骨肉瘤患者预后的独立保护因素(P<0.05)。结论肿瘤部位和病理性骨折是影响骨肉瘤患者预后的独立危险因素,营养水平是影响骨肉瘤患者预后的独立保护因素。
Objective To investigate influential factors of prognosis for patients with osteosarcoma. Methods A total of 95 patients with osteosarcoma admitted during January 2008 and July 2015 were recruited in this study. Possibly influential factors of prognosis such as gender, age, tumor site, operative process and nutrition level were extracted, and overall sur- vival rate was calculated by Kaplan-Meier method. The univariate analysis was performed by LogRank test, while the multiva- riate analysis was performed by muhielement Cox regression model. Results The 95 patients were followed-up for 13-90 months, and the average follow-up time was 41.3 months. During follow-up, 67 patients died, and 28 patients were survival. Median follow-up time was 39 months. The 3 and 5 years of overall survival rates were 52.6% and 33.7% respectively. Uni- variatc analysis showed that the prognosis of patients with osteosarcoma was closely correlated with tumor site, nutrition level, alkaline phosphatase level, tumor stage and pathological fracture （P 〈 0.05 ）. The muhielement COX regression analysis showed that tumor site and pathological fracture were independently risk factors, while nutrition level was independently protection factor of affecting prognosis for patients with osteosarcoma （P 〈0. 05）. Conclusion Tumor site and pathological frac- ture are independently risk factors, while nutrition level is independently protection factor of affecting prognosis for patients with osteosarcoma.
