摘要
利用现代信息技术及具有自主知识产权的各种家庭终端监测仪器如心电监测仪、血压仪、血氧饱和度、血糖仪等建立"家庭-社区-医院"云监测会诊平台,将每个家庭与社区及医院无缝对接,所有数据储存到每个慢性病患者的健康档案,对高血压病、冠心病、糖尿病、慢性阻塞性肺病等慢性疾病实施有效综合防控。在原有社区健康档案及信息系统的基础上,完善云健康档案及慢性病管理系统,让病人病历资料变成活的档案,实现对慢性病的实时监控、上级医院对社区医生的指导及双向转诊服务。将广州市海珠区沙园社区医疗卫生服务中心建成慢性病综合防控示范社区。
Using various modern families terminal monitoring instruments of modern information technology and independent intellectual property rights such as ECG monitor,blood pressure,oxygen saturation,blood glucose meter to establish＂home-community-hospital＂cloud monitoring consultation platform,each family and community hospitals and seamless docking,all data is stored in each patients＇ health records with the effective implementation of comprehensive prevention and control of chronic diseases,such as hypertension,coronary heart disease,diabetes,chronic obstructive pulmonary disease. Based on the original community health records and information system,improving the cloud health records and chronic disease management system,let the patient medical records into a living archive,to achieve real-time monitoring ofchronic diseases andreferral services and guidance between the senior hospital and community doctors. We believe the Shayuancommunity,Haizhu distinct,Guangzhou will be established into model community of chronic diseases comprehensive prevention.
