摘要
目的观察重度急性有机磷农药中毒合并急性呼吸衰竭患者ICU治疗效果。方法选取凉山州第一人民医院2013年1月—2015年1月收治的重度急性有机磷农药中毒合并急性呼吸衰竭患者128例,隨机分为对照组和观察组,各64例。对照组患者给予常规内科治疗,观察组患者给予ICU治疗,比较两组患者治疗效果。结果观察组患者临床疗效优于对照组(P<0.05);观察组患者阿托品用量少子对照组,阿托品化时间、有创通气时间、总机械通气时间及住院时间均短于对照组(P<0.05);观察组患者脑损伤、心脏损伤、阿托品中毒、中间综合征、气管重新插管及呼吸机相关性肺炎发生率低于对照组(P<0.05)。结论重度急性有机磷农药中毒合并急性呼吸衰竭患者ICU治疗效果确切,能減少患者住院时间,且并发症少。
Objective To observe the effect of ICU in treatment of severe acute organophosphorus poisoning combined with acute respiratory failure . Methods 128 severe acute organophosphorus poisoning patients combined with acute respiratory failure were selected in First people＇s Hospital of Liangshan from January 2013 to January 2015. Patients in the control group were given routine medical treatment, the observation group were given ICU treatment, and the treatment effect was compared between the two groups. Results The treatment effect of observation group was statistically significant better than of control group （P 〈 0. 05） ; atropine dosage of observation group was statistically significantly less than control group, atropine atropine time , invasive mechanical ventilation time , total mechanical venti lation time and hospitalization time of observation group were statistically significantly shorter than those of control group （P 〈 0. 05） ; the incidence of brain injury , cardiac injury , atropine poisoning, intermediate syndrome, tracheal intubation and venti lator associated pneumonia of observation group were statistically significant lower than those of control group （ P 〈 0. 05 ）. Conclusion Severe acute organophosphorus pesticide poisoning combined with acute respiratory failure patients with ICU treatment effect is exact, can reduce the hospital stay, and less complications.
