摘要
目的 探讨幽门螺杆菌感染和平均血小板体积(MPV)与冠状动脉粥样硬化性心脏病(CAD)的相关性.方法 根据冠脉造影(CAG)的结果,将292例研究对象分为对照组143例和冠心病组149例.冠心病组根据病变支数又分为单支病变组(38例)、双支病变组(46例)和多支病变组(65例).所有病例均行13C-呼气试验、血浆幽门螺杆菌抗体(IgG)测定及各组血液平均血小板体积,并计算冠心病组Gensini积分;同时收集病史、尿酸、血脂、高敏C反应蛋白、糖化血红蛋白等指标,评价幽门螺杆菌感染值、MPV与冠心病的相关性.结果 ①两组患者吸烟、年龄、低密度脂蛋白、高敏C反应蛋白、同型半胱氨酸比较,P值分别为0.003、0.011、0.011、0.007、0.001,差异均有统计学意义(P<0.05).②冠心病组幽门螺杆菌感染值14.23±1.28,MPV 9.96±0.68及Gensini积分43.17±12.42,明显高于对照组幽门螺杆菌感染值10.95±1.59,MPV 8.26±0.62及Gemini积分0,差异均有统计学意义(P<0.05).③幽门螺杆菌感染值(单支病变组12.88±0.53,双支病变组17.72±2.92,多支病变组21.27±1.35)、MPV(单支病变组8.82±0.66,双支病变组9.86±0.71,多支病变组10.99±0.62)及Gensini积分(单支病变组11.60±4.14,双支病变组42.67±8.11,多支病变组72.20±7.81)随着病变血管数增加而升高,各组间比较差异均有统计学意义(P<0.05).④幽门螺杆菌感染值与Gensini积分呈正相关(r=0.869,P<0.01),MPV与Gensini积分亦呈正相关(r=0.881,P<0.01),幽门螺杆菌感染值与MPV呈正相关(r=0.853,P<0.01).结论 幽门螺杆菌感染值和MPV对冠心病有一定的预测价值.
Objectiv To investigate the correlation between Helicobacter pylori infection and coronary artery disease(CAD) as well as mean platelet volume(MPV). Method According to the results of coronary angiography (CAG), the patients were divided into control group (n=143)and CAD group(n=149). According to the number of vessels involved, CAD group was subdivided into one-vessel group (n=38), two-vessel group (n=46), multi-vessel group (n=65). All the patients underwent 13C-breath test, Helicobacter pylori IgG antibody determination and MPV, at the same time, the patien’s case history was colleted, microallbuminuria (UMA),high sensitivity C-reactive protein(Hs-CRP) were detected and blood routine test, biochemical parameter examination, glycosylated hemoglobin test were performed to evaluate the correlation between Helicobacter pylori and MPV as well as CAD. Results 1.There were significant difference in smoking, age, low density lipoprotein cholesterol (LDL-c), Hs-CRP, homocysteine (HCY) between the two groups. 2. The Helicobacter pylori infection level , MPV and Gensini’s score in CAD group were higher than those in control group significantly, (p〈0.05). 3.The Helicobacter pylori infection level, MPV and Gensini , s score were significantly increased with the number of vessels involved,(p〈0.05).4.We also found that Helicobacter pylori infection were positively correlated with the Gensini , s score(r=0.869,P〈0.01), and the MPV were also positively correlated with the Gensini , s score(r=0.881,P〈0.01). The Helicobacter pylori infection level were positively correlated with MPV(r=0.853,P〈0.01). Conclusion Helicobacter pylori infection and heme oxygenase-1 have some predictive value to CAD.
