摘要
目的探讨"三人行"延续护理模式对初始使用基础胰岛素患者出院后的综合管理实施效果。方法本院2014年4—10月收治的2型糖尿病患者120例,随机平均分为两组,其中观察组60例,在住院常规治疗的基础上出院后给予"三人行"延续护理模式干预;对照组60例,只给予常规的住院基础治疗并不采取出院延续护理。比较两组患者干预前后的各项生化指标、胰岛素应用及注射相关问题的认知情况,发放"我对胰岛素的看法"(DAWN)量表进行评估。结果对照组干预前后,各项指标差异无统计学意义,观察组干预前后差异显著(P<0.01),且干预后,观察组各项生化指标、生活管理及对胰岛素的认知、副作用、注射相关问题等方面优于对照组,差异均具有显著性(P<0.01)。结论 "三人行"延续护理模式对初始使用基础胰岛素患者具有良好的干预效果,提高了患者对胰岛素治疗的依从性,是临床开展胰岛素使用教育的有效途径。
Objective To investigate the implementation effect of the ＂ Sanrenxing extended care model＂ management intervention on the basis of initial use of insulin for diabetes patients discharged home. Methods120 cases of type 2 diabetes patients treated in our hospital from April 2014 to October 2014 were randomly divided into two groups with 60 cases in each group,the observation group,on the basis of hospital discharge given conventional treatment,gave the ＂ three line extended care ＂; the control group received conventional treatment. Compared with the two groups of patients before and after the intervention of the biochemical indicators,insulin application and injection related issues of cognition,released ＂ my perception of insulin＂（ DAWN） scale to evaluate.Results Intervention in the control group,the indicators difference was not statistically significant,before and after the intervention group were significantly different（ P〈0. 01）,after the intervention,compared with the control group,the biochemical indexes,the life management of the observation group and the cognition,side effects and injection related problems of the observation group were better than those of the control group（ P〈0.01）. Conclusions The ＂ Sanrenxing extended care model＂ has good effect on the initial use of insulin in patients with basal insulin therapy,and it is an effective way to develop insulin in clinical application.
