摘要
目的检测冠心病(CHD)患者血清Afamin浓度水平,并探讨Afamin在冠心病中的临床意义。方法酶联免疫反应方法检测167例CHD患者(CHD组)及127例健康对照者(对照组)血清Afamin水平,同时收集所有受试者血糖、血脂等临床资料。结果 CHD组血清Afamin浓度明显高于对照组(417.78±55.87vs231.67±71.71,P<0.01)。相关分析表明,CHD组血清Afamin水平与MDA(r=0.271,P=0.011)、hs-CRP(r=0.191,P=0.021)、TNF-α(r=0.212,P=0.034)、HOMA-IR(r=0.161,P=0.021)水平呈正相关,但与HDL-C(r=-0.191,P=0.029)、GSH(r=-0.376,P<0.01)水平呈负相关;多重线性回归分析提示,MDA和HOMA-IR是影响Afamin水平的独立相关因素;血清Afamin诊断冠心病ROC曲线下面积为0.957,最佳截断点为460.11 ng/ml。结论血清Afamin与胰岛素抵抗、氧化应激密切相关,有助于冠心病诊断以及病情检测。
Objective To detect the levels of serum Afamin in patients with coronary heart disease (CHD), and to analyze the clinical significance of Afamin in CHD. Methods Enzyme linked immunosorbent assay (ELISA) was used to detect the levels of serum Afamin in CHD patients (the CHD group, n=167) and healthy persons (the control group, n=127). Such clinical data as blood glucose and blood lipid of all subjects were collected, Results The concentration of serum Afamin in the CHD group was much higher than that in the control group(417.78±55.87 vs 231.67±71.71; P 〈 0.01). As indicated by relevant analysis, the levels of serum Afamin in the CHD group was positively correlated to the levels of MDA (r=0,271, P= 0.011), hs-CRP (r= 0.191, P= 0.021), TNF-α (r = 0.212, P = 0.034) and HOMA-IR (r = 0.161, P = 0.021) but negatively correlated to the levels of HDL-C (r = -0.191, P = 0.029) and GSH (r = -0.376, P 〈 0.01); multiple linear regression analysis indicated that MDA and HOMA-IR were independent related factors affecting the levels of Afamin; the area under the ROC curve for the diagnosis of CHD with Afamin was 0.957, and the best cut-off point was 460.11 ng,/m,Conclusion Serum Afamin is closely related to oxidative stress, inflammation and insulin resistance in CHD. It is useful for the diagnosis and disease surveillance of CHD.
