摘要
目的检测冠心病患者血清脂质运载蛋白-2(Lipocalin-2)浓度水平,探讨脂质运载蛋白-2在冠心病中的临床意义。方法用酶联免疫反应方法检测冠心病患者167例(CHD组)及127名健康对照者(对照组)血清脂质运载蛋白-2水平,同时收集血糖、血脂等-般性资料。结果CHD组患者血清脂质运载蛋白-2浓度明显高于对照组[(417.78±55.87)pg/ml比(231.67±71.71)pg/ml,P=0.000]。相关分析表明,脂质运载蛋白-2与MDA(r=0.257,P=0.006)、hs—CRP(r=0.178,P=0.018),TNF—d(r=0.245,P=0.005)、HOMA—IR(r=0.214,P=-0.001)呈正相关,与HDL—c(r=-0.184,P=-0.027)、GSH(r=-0.358,P=0.000)呈负相关。多重线性回归分析提示,MDA、HOMA—IR是影响脂质运载蛋白-2水平的独立相关因素,血清脂质运载蛋白-2诊断冠心病ROC曲线下面积为0.812(0.765,0.861)。结论血清脂质运载蛋白-2与胰岛素抵抗、氧化应激及炎症密切相关,它有助于冠心病诊断及病情检测。
Objective To detect the serum levels of lipocalin-2 in patients of coronary heart diease (CHD), and to analysis clinical value in coronary heart disease. Methods Enzyme linked immunosorbent assay (ELISA) was used to detect the serum lipocalin-2 in 167 CHD patients and 127 healthy controls. Results The serum Lipocalin-2 concentration in coronary heart disease was significantly higher than in healthy controls [(417.78±55.87)pg/ml vs (231.67±71.71)pg/ml, P=0.000]. Lipocalin-2 associated positively with MDA (r= 0.257, P=0.006), hs-CRP (r=0.178, P=0.018), TNF-a (r=0.245, P=0.005), HOMA-IR (r=0.214, P= 0.001), but negatively with(r=-0.184, P=-0.027), GSH(r=-0.358, P=0.000). The area under the receiver oper- ating characteristic curve for CHD diagnosis was estimated to Be 0.812 (0.765, 0.861 ). Conclusion Lipocalin-2 is closely related to oxidative stress, inflammation and insulin resistance in CHD. It is useful for disease surveil- lance and diagnosis of CHD by serum lipoealin-2.
