摘要
目的 探讨腔内超声联合CEA、CA199对直肠癌术前分期的诊断价值。方法 选择110例行手术治疗的直肠癌患者,术前1周所有患者行直肠TRUS检查,以判断超声分期,TRUS后1~3天检测血清CEA、CA199水平,对照TRUS、TURS联合CEA、CA199术前分期结果与术后病理分期的结果。结果 TRUS诊断总准确率为76.4%,CEA、CA199水平随病理分期的升高逐渐升高。病理分期T1~T4期术前血清CEA、CA199阳性率比较差异有统计学意义,P均〈0.05。TRUS联合CEA、CA199对直肠癌术前分期的准确率达90.9%,明显高于TRUS单独诊断准确率,P〈0.05。结论 术前血清CEA、CA199检测水平对直肠癌TRUS分期有参考意义,TRUS联合血清CEA、CA199检测可以提高直肠癌术前分期的准确率。
Objective To investigate the diagnostic analysis of intraductal-uhra sonography combined with CEA,CA199 on preoperative staging of colon cancer. Methods 110 rectal carcinoma treated with surgery were given rectum TRUS check in order to judge ultrasonic staging, CEA,CA199 level in serum 1-3 day after TURS, the preoperative staging result with postoperative pathologic staging of TRUS,TURS CEA,CA199 were compared. Results The total diagnosis accuracy of TRUS was 76.4%, the CEA,CA199 level were raised along with the increase of pathological staging. The positive rate of CEA,CA199 in preoperative serum of pathologic T1-T4 had significant difference,P 〈0.05. The accurate rate of TRUS combined with CEA,CA199 on colon cancer was 90.9%, which was obvious higher than TRUS diagnosis accurate rate,P 〈 0.05. Conclusion CEA,CA199 in preop- erative serum had referential meaning for colon cancer TURS staging,TRUS combined with serum CEA,CA199 can improve the accurate rate of preoperative staging of rectal cancer.
