摘要
目的观察ω-3鱼油脂肪乳联合益气活血中药对营养风险筛查(NRS)评分≥3分的消化道肿瘤患者术后胃肠激素及胃肠功能恢复的影响。方法将118例NRS评分≥3分的消化道肿瘤术后患者随机均分为观察组59例和对照组59例。2组均给予西医常规营养支持治疗,观察组在此治疗基础上给予ω-3鱼油脂肪乳静脉滴注+自拟益气活血方剂胃管内给药,连用至术后7 d。观察2组治疗前及治疗3 d、7 d后胃肠激素指标[血清胃泌素(GAS)、胃动素(MTL)、生长抑素(SS)、血管活性肠肽(VIP)]、消化道症状(腹胀、腹痛、恶心、食后饱胀、嗳气、乏力)评分、胃肠道黏膜屏障指标[血浆D-乳酸、肠脂肪酸结合蛋白(IFABP)]及营养指标[血清白蛋白(ALB)、前白蛋白(PA)]变化情况。结果 2组治疗3 d、7 d后血清GAS、MTL水平均明显降低(P均<0.05),但观察组上述指标水平均明显高于对照组(P均<0.05);2组治疗3d、7 d后血清SS、VIP水平均明显升高(P均<0.05),但观察组上述指标水平均明显低于对照组(P均<0.05);2组治疗3 d、7 d后腹胀、腹痛、恶心、食后饱胀、嗳气、乏力评分均明显降低,且观察组各项评分均明显低于对照组(P均<0.05);2组治疗3 d、7 d后血浆D-乳酸、I-FABP水平均明显降低(P均<0.05),而血清ALB、PA均明显升高(P均<0.05),且观察组上述指标改善更明显(P均<0.05)。结论ω-3鱼油脂肪乳联合益气活血中药能够显著调节NRS评分≥3分的消化道肿瘤术后患者胃肠激素的表达,改善患者消化道症状及营养状况,保护胃肠道黏膜屏障。
Objective It is to observe the influence of ω-3 fish oil fat emulsion combined with traditional Chinese medicine for invigorating qi and promoting blood circulation on gastrointestinal hormones and postoperative recovery of gastrointestinal function in digestive tract cancer patients with NRS of 3 or more. Methods One hundred and eighteen cases of digestive tract cancer postoperative patients with NRS of 3 or more were randomly divided into observation group (59 cases) and control group (59 cases). All patients were treated with conventional western medicine nutrition support, and plus ω- 3 fish oil fat emulsion injection + invigorating qi and promoting blood circulation prescription enteral nutritioh therapy in the observation group, continuous application to the seventh day after operate. The gastrointestinal hormone indicators[ serum gastrin (GAS), motilin ( MTL), somatostatin ( SS), vasoactive intestinal peptide (VIP) ], gastrointestinal symptoms score ( abdominal distension, abdominal pain, nausea, bloating after eating, belching, fatigue), gastrointestinal mucosal barrier index (plasma D - lactate, I - FABP) and nutritional indicators (Serum albumin, pre albumin) were observed in the two groups before treatment and after treatment for 3 days, 7 days. Results The levels of serum GAS and MTL were significantly lower in the both groups after treatment for 3 and 7 days ( all P 〈 0.05) , and those of the observation group were significantly higher than the control group (all P 〈 0. 05) ; the levels of SS, VIP were significantly increased in the both groups after treatment for 3 and 7 days ( all P 〈 0.05 ), and those of the observation group were significantly lower than the control group ( all P 〈 0. 05 ) ; the scores of gastrointestinal symptoms such as abdominal distension, abdominal pain, nausea, bloating after eating, belching, fatiguewere significantly lower in the both groups after treatment for 3 and 7 days ( all P 〈 0.05) , and those o
