盐酸右美托咪定用于小儿扁桃体切除术前镇静的效果与安全性 被引量:4

Effect and Safety of Dexmedetomidine Hydrochloride in Preoperative Sedative of Pediatric Tonsillectomy Gland Surgery

摘要 目的:探讨盐酸右美托咪定(DEX)应用于小儿扁桃体切除术前镇静的效果与安全性。方法:选取2012年2月至2015年2月在我院行扁桃体切除术的2~10岁儿童85例,根据术前镇静用药分为DEX组45例和氯胺酮组40例,比较分析两组患儿的术前镇静效果、术中临床指标、苏醒时间、术后不良反应等。结果:DEX组术前镇静效果优于氯胺酮组,插管后及拔管后10 min的血压、心率均高于氯胺酮组,拔管恢复时间、呼吸恢复时间、意识恢复时间均短于氯胺酮组(P均〈0.05)。两组术后哭闹发生率比较差异无统计学意义(P〉0.05);DEX组术后躁动、恶心、呕吐的发生率均低于氯胺酮组(P均〈0.05)。结论:DEX可安全应用于小儿扁桃体切除术前镇静,可降低术后躁动、恶心、呕吐等不良反应的发生率。 Objective: To explore the effect and safety of dexmedetomidine( DEX) hydrochloride in preoperative sedative of pediatric tonsillectomy gland surgery. Methods: Randomly selected 85 cases of 2 to 10 years old children with tonsillectomy gland surgery of the hospital from February 2012 to February 2015. According to the drug use,all children were divided into DEX group( 45 cases) and ketamin group( 40 cases). Sedative effect,clinical indicators,awakening time,and postoperative adverse reactions in two groups were compared. Results: The sedative effect of the DEX group before surgery was better than that of the ketamin group( P〈0. 05). The blood pressure and heart rate after intubation and 10 minute after extubation of the DEX group were higher than those of the ketamin group( P〈0. 05). Recovery time of extubation,breathe and consciousness in the DEX group were shorter than those of the ketamin group( P〈0. 05). There was no difference in the incidence of postoperative crying between the two groups( P〉0. 05). In DEX group,the incidence of postoperative agitation,nausea,vomiting were lower than ketamin group( P〈0. 05). Conclusion: Dexmedetomidine hydrochloride can be safely used in children with tonsillectomy gland surgery sedation,and can reduce the incidence of postoperative adverse reactions,as agitation,nausea,vomiting.
