摘要
目的了解我国肺结核患者家庭密切接触者活动性肺结核检出情况，估算活动性肺结核检出率。方法系统检索中国全文数据库总库（CNKI）、万方数据库（Wanfangdata）、维普数据库（VIP）、中国生物医学文献数据库（CBM），以及3个外文数据库PubMed、web of Science和Embase 2005年1月1日至2016年3月31日有关涂阳肺结核患者密切接触者活动性肺结核检出情况，且研究现场为中国大陆地区的相关文献，共检出文献3939篇，最后纳入18篇文献进行分析，依据加强流行病学观察性研究的报告（strengthening the reporting of observational studies in epidemiology，STROBE）声明要求对文献进行质量评价，采用广义倒方差模型的方法估算家庭密切接触者活动性肺结核检出合并率，以及按不同性别、年龄、地区和样本来源进行亚组分析；并进行敏感性分析，评价文献质量对结果的影响。结果18篇文献中，家庭密切接触者筛查数为44495名，共检查出活动性肺结核患者1517例，活动性肺结核检出的合并率为3．58％（95％CI=3．24％-3．96％）；其中〉60岁组的合并率（6．02％）较15-岁组（2．94％）及〈15岁组（2．45％）高，差异具有统计学意义（F=16．58，P=0．001）。结论我国涂阳肺结核家庭密切接触者活动性肺结核检出率较高，疫情不容忽视，需要加大对肺结核家庭密切接触者的筛查力度，同时做好家庭密切接触者的结核病防治宣传教育工作，提高对结核病的防范意识。
Objective To understand and estimate the yield of active tuberculosis cases among household con- tacts of pulmonary tuberculosis （PTB） patient. Methods A total of 3939 related literature regarding the yield of active tuberculosis cases among household contacts of PTB published in CNKI,Wanfang Database, Chongqing VIP, CBM, PubMed, Web of Science and Embase published between 1st Janurary 2005 and 31st March 2016 were retrieved. Among them, eighteen papers were included for our Meta-analysis in the end. Related information was extracted and the generic inverse variance model was applied to estimate the following parameters, including detection rate, di{ferences on genders, ages, locations and origin of the samples. Quality of the study was evaluated through the STROBE statement and sensitivity analysis was performed to assess the impact of the quality. Results In our study, a total of 44 495 household contacts were screened, and 1517 patients were detected. The overall yield was 3.58% （95%CI=3. 24%-3. 96%）. For age above 60 （6.02%）, yield was higher than that of age below 15 and age 15 to 60 （2.45% and 2.94- respectively） ,with significant differences （F=16. 58,P〈0. 001）. Conclusion The yield of active tuberculosis cases among household contacts of sputum-smear-positive PTB was high in China, and the epidemic situation was worth to concern. We should strengthen the efforts to screen the household contacts and educate them the knowledge of preventing the tuberculosis, so as to improve the awareness of prevention.
