多发性骨髓瘤合并肾损害的相关因素分析 被引量:5

摘要 目的探讨导致多发性骨髓瘤(MM)合并肾损害的相关因素。方法选取2010年1月至2015年1月间重庆市梁平县人民医院收治的142例MM患者,根据患者有无合并肾损伤情况,分为肾损伤组(70例)和非肾损伤组(72例)。比较两组患者的一般临床情况和生化指标,包括血β_2微球蛋白(β_2-MG)、血尿酸(UA)、血钙(Ca)和血红蛋白(Hb)等,对各项指标进行统计学分析。采用Logistic回归法,分析导致MM合并肾损伤发生的危险因素。结果肾损伤组患者年龄、β2-MG、UA和Ca水平均显著高于非肾损伤组,Hb水平则明显低于非肾损伤组,差异均有统计学意义(均P<0.05)。Logistic回归显示,发现影响MM合并肾损伤发生的危险因素为高龄、高钙血症、贫血和高尿酸血症等,差异有统计学意义(P<0.05)。结论 MM合并肾损害是MM患者较为多见的表现形式,其中高龄、高钙血症、贫血和高尿酸血症MM患者更易发生肾损伤。 Objective To explore the risk factors of multiple myeloma( MM) complicated with renal impairment. Methods 142 MM patients were selected in Liangping People's Hospital from January2010 to January 2015. Patients were divided into injury group( 70 cases) and non-injury group( 72 cases)according to the incidence of renal impairment. The general clinical condition and biochemical index including β2-microglobulin( β2-MG),uric acid( UA),calcium( Ca) and hemoglobin( Hb) were compared between groups. Using Logistic regression,the risk factors of MM complicated with renal impairment was analyzed. Results Age,levels of β2-MG,UA and Ca in injury group were significantly higher than those of non-injury group; Level of Hb in injury group was lower than that of non-injury group,with significant differences between groups( P〈 0. 05). Logistic regression analysis revealed that risk factors of MM complicated with renal impairment were older age,hypercalcemia,anemia and hyperuricemia( P〈 0. 05).Conclusion MM complicated with renal impairment has a higher prevalence in MM patients,while patients with older age,hypercalcemia,anemia and hyperuricemia have a higher risk of renal impairment.
