摘要
目的:了解武汉科技大学附属天佑医院(以下简称"我院")骨科Ⅰ类切口手术围术期预防性使用抗菌药物的情况,找出所存在的问题,提出改进措施。方法:回顾性调查分析2013—2015年我院骨科1 271例Ⅰ类切口手术患者围术期抗菌药物预防性使用率、给药时间合理率、品种选择合理率、用药疗程和联合用药情况,其中2013年377例、2014年405例、2015年489例。结果:2013—2015年我院骨科Ⅰ类切口手术患者围术期抗菌药物预防性使用率分别为96.02%(362/377)、78.27%(317/405)、59.92%(293/489),给药时间合理率分别为74.86%(271/362)、90.54%(287/317)、97.27%(285/293),品种选择合理率分别为58.01%(210/362)、76.97%(244/317)、93.17%(273/293),抗菌药物平均用药疗程分别为(8.5±1.6)、(5.7±1.5)、(3.3±1.1)d。结论:我院骨科Ⅰ类切口手术围术期预防性应用抗菌药物的各项指标总体呈下降趋势,但抗菌药物预防性使用率、预防性用药疗程仍未达到要求,仍需加强规范骨科Ⅰ类切口手术围术期抗菌药物的预防性应用。
OBJECTIVE： To investigate the prophylactic application status of antibiotics in 1 271 patients with type Ⅰ incision in department of orthopaedics during perioperative period（ hereinafter referred to as＂our hospital＂）,so as to find out the existing problems and put forward improvement measures. METHODS： Retrospective analysis was conducted on the prophylactic application of antibiotics in 1 271 patients with type Ⅰ incision in department of orthopaedics during perioperative period during 2013-2015 in terms of the prophylactic application rate,the rational rate of administration time and categories selection,treatment course and drug combination,among which,there were377 cases in 2013,405 cases in 2014 and 489 cases in 2015. RESULTS： During 2013-2015,the prophylactic application rate were respectively 96. 02%（ 362 /377）,78. 27%（ 317 /405） and 59. 92%（ 293 /489）,the rational rate of administration time were respectively 74. 86%（ 271 /362）,90. 54%（ 287 /317） and 97. 27%（ 285 /293）,the rational rate of categories selection were respectively 58. 01%（ 210 /362）,76. 97%（ 244 /317） and 93. 17%（ 273 /293）,and the average treatment course were respectively（ 8. 5 ± 1. 6） d,（ 5. 7 ± 1. 5） d and（ 3. 3 ± 1. 1） d.CONCLUSIONS： The indicators of prophylactic application of antibiotics in type Ⅰ incision showed downward trend in our hospital,yet the prophylactic application rate and treatment course are still blew the required standard. It is necessary to strengthen the prophylactic application of antibiotics in patients with type Ⅰ incision in department of orthopaedics during perioperative period.
