摘要
目的:探讨医用几丁糖在妇产科术后防止组织黏连的临床疗效。方法:选取2011年1月—2014年1月成都市妇女儿童中心医院收治的行妇产科手术的患者461例,以随机数字表法分为观察组254例和对照组207例。观察组患者于手术结束前将医用几丁糖涂抹于手术创面及腹腔内,对照组患者未使用医用几丁糖。观察2组患者术后1周基本情况;观察患者术后6个月、1年的慢性盆腔痛、腹膜囊肿、肠梗阻情况;术后2年内,观察患者手术的疗效。结果:观察组患者术后6个月内、6月—1年内,慢性盆腔痛、腹膜囊肿、不完全肠梗阻的发生率均低于对照组,差异有统计学意义(P<0.05);术后2年内,观察组患者的手术疗效高于对照组,差异有统计学意义(P<0.05)。结论:医用几丁糖预防术后组织黏连,安全、有效。
OBJECTIVE：To probe into the clinical efficacy of medical chitosan in prevention of postoperative tissue adhesion in department of gynecology and obstetrics. METHODS： 461 patients undergoing gynecologic surgery admitted into Chengdu Women and Children＇s Center Hospital from Jan.2011 to Jan.2014 were selected to be divided into observation group（254 cases） and control group（207 cases） via the random number table.The observation group were injected chitosan in wound surface and around abdominal cavity, and the control group were closed up the incision without any prevention.The conditions after surgery of 1 week was observed.The chronic pelvic pain, peritoneal cysts, intestinal obstruction in two groups were observed after surgery of 6 months, 1 year and 2 years.The efficacy of surgery was observed in the follow-up period of 2 years.RESULTS：The incidence of chronic pelvic pain,peritoneal cysts, intestinal obstruction in observation group were lower than that of control group after surgery of 6 months, 1 year and 2 years（ P〈0.05） , and the efficacy of observation group was better than that of control group, with statistical significance in the follow-up period of 2 years （P〈0.05）.CONCLUSIONS：Medical chitosan can be safely and effectively used in prevention of postoperative tissue adhesion.
