摘要
目的:观察小剂量氯胺酮预防剖宫产术后抑郁的临床效果。方法:60例ASAⅠ~Ⅱ级行择期剖宫产术的孕妇,随机分为对照组和观察组,每组30例。术前1 d采用医院焦虑抑郁量表(hospital anxiety and depression scale,HADS)评价产妇抑郁状况,术中观察组在胎儿娩出后静注氯胺酮(0.5 mg/kg),对照组在胎儿娩出后静注生理盐水,观察并记录不良反应的发生情况。术后4 d采用产后抑郁筛查量表(postpartum depression screening scale,PDSS)评价产妇的抑郁状况。结果:两组产妇术前1 d的HADS分值、抑郁症和术中不良反应的发生率差异无统计学意义(P〉0.05)。观察组术后4 d分值(57.07±13.97)明显低于对照组(71.57±30.49),观察组术后4 d抑郁发生率为6.67%,对照组为30%,两组比较差异均有统计学意义(P〈0.05)。结论:剖宫产术中静注小剂量氯胺酮可降低产后抑郁症的发生率,对产后抑郁症具有一定的预防作用。
Objective:To observe the effect of low-dose ketamine on prevention of depression after cesarean section. Methods: Sixty patients who were underwent elective cesarean section with ASA I-II were divided randomly into two groups: study group and control group and each group included 30 cases. The situation of depression in patients were evaluated by Hospital Anxiety and Depression Scale (HADS) on one day before the operation. The study group was given ketamine(0.5mg/kg) with intravenous injection after the fetuses were delivered, while the control group was given normal saline to instead, observe and record the occurrence of adverse reactions. Four days after the operation,the status of depression in patients were evaluated by Postpartum Depression Screening Scale (PDSS) . Results:There were no differences in the HADS scores on one day before operation, including the incidence of depression, the incidence of adverse effects between two groups(P 〉 0.05 ). The PDSS score on the 4tb day after operation (57.07± 13.97) in the study group was significantly lower than that in the control group (71.57 ± 30.49). The incidence of depression on the 4th day after operation in the study group was 6.67% , and the control group was 30% , the difference of the two groups was statistically significant (P 〈 0.05 ). Conclusion:Low-dose ketamine given in cesarean section can reduce the incidence of postpartum depression, and it has certain effect to prevent postpartum depression.
