摘要
目的:探讨胎儿纤维连接蛋白(f FN)测试条联合宫颈长度在提高早产预测价值的应用。方法:回顾性分析2011年6月-2015年8月本院孕28~35周有先兆早产征象的产妇168例,其中早产72例,足月产96例,所有产妇均于临产前通过f FN测试条检测f FN分泌情况,以及通过阴道B超检测宫颈长度,统计分析两种检测方法及其联合检测对早产的预测情况。结果:宫颈长度检测法(以〈30 mm为界限)预测早产的敏感度、特异度、准确度分别为77.78%、65.63%、70.83%;f FN测试条检测法预测早产的敏感度、特异度、准确度分别为83.33%、79.17%、80.95%;联合检测法预测早产的敏感度、特异度、准确度分别为93.06%、89.58%、91.07%;在预测敏感度、特异度、准确度方面,联合检测法均优于f FN测试条检测法,f FN测试条检测法均优于宫颈长度检测法,差异均有统计学意义(P〈0.05)。结论:f FN测试条与宫颈长度对早产均有一定的预测意义,而两者联合检测对早产预测具有更为良好的敏感度、特异度、准确度,可作为预测早产的重要方法,值得临床作进一步推广。
Objective:To discuss the application of fetal protein fiber connection(f FN) test strip joint cervical length in improving the predictive value of premature.Method:A total of 137 women in 28-35 weeks had premonitory signs of premature were retrospectively analyzed from June 2011 to 2011 in our hospital.72 cases were premature birth,from 96 cases of full term,all maternal article were tested by f FN detection secretion f FN prior to labor,and through the vaginal ultrasound examination cervical length,analysed two kinds of detection methods and their joint detection for the forecast of preterm birth.Result:The cervical length test(with30 mm line) method for respectively forecast the premature delivery of the sensitivity,specific degree, accuracy were 77.78%,65.63% and 70.83%.Article f FN test method for respectively forecast the premature delivery of the sensitivity,specific degree, accuracy were 83.33%,79.17% and 80.95%.The joint test tmethod for respectively forecast the premature delivery of the sensitivity,specific degree,accuracy were 93.06%,89.58% and 91.07%.In terms of forecast accuracy sensitivity,specific degree,the joint method were better than the f FN test article,article f FN test method were better than the cervical length test,the differences were statistically significant(P〈0.05).Conclusion:FFN test strip and the cervical length of preterm birth all have certain prediction significance, and the joint detection for preterm birth prediction with better sensitivity and accuracy of the specific degree, and can be used as a prediction of the important methods for preterm birth,it's worth for further clinical promotion.
