胰腺无功能神经内分泌肿瘤与实性假乳头状瘤的多层螺旋CT增强特征比较 被引量:10

Comparison of Multi-slice Spiral CT Enhanced Features of Non-functional Pancreatic Neuroendocrine Tumors with Solid Pseudopapillary Tumors

摘要 目的比较胰腺无功能神经内分泌肿瘤(NFPNET)与实性假乳头状瘤(SPT)的多层螺旋CT双期增强特征。资料与方法回顾性分析经病理证实的NFPNET 16例和SPT 16例,行CT平扫及双期增强扫描,比较两种肿瘤的动脉期和门静脉期强化幅度绝对值、双期增强CT值高于或低于胰腺实质的数量、强化峰值出现时相、强化方式、是否有周边侵犯或远处转移。结果 NFPNET组与SPT组动脉期强化绝对值分别为57.2±36.9与18.3±15.2,门静脉期分别为53.6±26.4与33.1±16.1,两者差异有统计学意义(P<0.05)。NFPNET组8例动脉期强化程度超过胰腺实质,SPT组0例;NFPNET组10例门静脉期强化程度超过胰腺实质,SPT组2例,两者差异有统计学意义(P<0.05);NFPNET组和SPT组在强化峰值出现时相、腊肠样强化、壁结节样强化及是否有周边侵犯和远处转移方面比较,差异均有统计学意义(P<0.05)。结论 NFPNET双期均为富血供增强,强化高于胰腺实质、强化峰值在动脉期、壁结节样强化较具特征性;而SPT强化程度低于胰腺实质、强化峰值在门静脉期、典型者可出现腊肠样强化,双期增强有助于两者的鉴别。 Purpose To compare double phase enhanced multi-slice spiral CT(MSCT) features of non-functional pancreatic neuroendocrine tumor(NFPNET) with those of solid pseudopapillary tumors of pancreas(SPT). Materials and Methods Sixteen cases of pathologically proven NFPNET and 16 cases of SPT were retrospectively analyzed. CT plain scan and double phase enhancement examination were granted. Compared the absolute value of relative enhancement between the two different tumors' arterial and portal phase, as well as degree of the two tumors' dual-phase enhanced CT value(higher or lower than that of the pancreatic parenchyma); observed the enhancement peak phase, the enhancement mode and whether accompanied with peripheral viscera invasion and distant metastasis. Results The absolute value of arterial phase enhancement in NFPNET and SPT was 57.2±36.9 and 18.3±15.2, respectively, portal phase enhanced CT value was 53.6±26.4 and 33.1±16.1, respectively. There was a significant difference between the two groups in the absolute value of both arterial and portal phase(P〈0.05). Eight cases of NFPNET and 0 case of SPT arterial enhancement degree was higher than that of the pancreatic parenchyma, 10 cases of NFPNET and 2 cases of SPT portal phase enhancement degree was higher than that of the pancreatic parenchyma. There was significant difference between the two groups in the dual-phase enhancement degree(P〈0.05). There was significant difference between the two groups in the enhancement peak phase, sausage shaped and wall nodular enhancement, as well as whether accompanied with peripheral invasion and distant metastasis(P〈0.05). Conclusion NFPNET shows rich blood supply enhancement in dual-phase, higher arterial phase enhancement degree than that of pancreatic parenchyma, enhancement peak in arterial phase and characterized by the wall nodular enhancement; and SPT shows lower portal phase enhancement degree than that of pancreatic parenchyma, enhancement peak in portal phase and characterize
